Sets out how to use six key principles for engaging with people and communities in order to create a person-centred, community-focussed approach to health, wellbeing and care. For each principle, the document demonstrates why it is important, provides a guide to putting it into practice, and offers case study examples of where it has been done well. Links to additional resources are also provided. The principles have been developed by the People and Communities Board, with the new models of care ‘vanguards’ sites, to support services in delivering new relationship with people and communities as set out in the Five Year Forward View. The principles cover: care and support is person-centred: personalised, coordinated, and empowering; services are created in partnership with citizens and communities; there is a focus on equality and narrowing inequality; carers are identified, supported and involved; volunteering and social action are key enablers; and voluntary community and social enterprise, and housing sectors are involved as key partners and enablers. The document is particularly aimed at leaders, managers and practitioners in the NHS and national and local government who are involved in improving and transforming health and care services in England. It will also be useful for patients, carers and the public.
