This report summarises the findings from a series of visits to a sample of clinical commissioning groups (CCGs) carried out by NHS England's National Clinical Director for Dementia with support from NHS Improving Quality . The purpose of the visits was to help build a national picture of what was working well in improving timely diagnosis of dementia and post-diagnostic support, to improve understanding about the challenges faced by CCGs and whether there were any common factors influencing diagnosis rates. The report highlights that those CCGs making greater progress in improving dementia diagnosis rates appear to have some common features: a focused, and clearly led plan of work to improve dementia care; commissioners and clinical leaders are active and visible in this pursuit, knowledgeable about what works, and proactively delivering a comprehensive strategy and action plan; work is proactive, systematic and sustained, rather than reactive and piecemeal; dementia care is being mainstreamed within existing health and care services; ambitions to improve diagnosis are being progressed within the wider context of raising awareness about dementia, improving knowledge and skills of staff; and voluntary and community sector organisations are key partners in the development of strategic plans and as service providers (Edited publisher abstract)

In the summer of 2007 the first graduates of the new degree in social work entered the workforce. The reform of social work training that led to the introduction of the degree was based on a set of principles, designed to create a continuous system of training and development and included the commitment to develop a model for the first year in practice. Newly qualified social workers are a precious resource for users of services and for employers, so it is crucial that the first year in practice is planned for and well managed. In recognition of how important it is that newly qualified social workers make a smooth transition from student to confident and competent practitioners and to fulfil the commitment made as part of the reform we are issuing this guidance to support all social work employers to develop a model of support for their first year in practice. Both employers and employees will be responsible for implementing the model. This document is issued by the Welsh Assembly Government as Good Practice guidance aimed at all employers of social workers. CSSIW will expect to see evidence of its adoption by Local authorities, through its inspection and review processes. The guidance is issued under Section 92 of the Health and Social Care Act 2003, and under Section 8 of the Care Standards Act 2003. Local Authorities are expected to commend these standards of practice to other organisations who employ social workers.

One of 20 targeted reviews of local authority areas looking specifically at how people move through the health and social care system, with a focus on how services work together. The review found that there was a strong ambition for partner agencies to work together and provide excellent services to the people of Oxfordshire but there was a lack of strategic planning. Despite this, health and social care professionals were highly dedicated to supporting people using services, their families and carers. While Oxfordshire has a history of public engagement, feedback has sometimes indicated that this has not always been effective. This has been recognised by engagement leads and a dedicated team has been set up to progress on the work required. The Social Care Institute of Excellence has reviewed the work of this team and confirmed that there is positive work taking place. Some of the key findings of the review were: the Adult Social Care Outcomes Framework for 2016/17 showed that the percentage of older people who were satisfied with their care and support was slightly above average, but there was mixed feedback from people and carers about the quality of their experiences when we spoke with them; plans were in place to build and adapt properties that could remain a person’s home for life and support longer term independence; system leaders and frontline workers reported widespread issues in respect of recruitment and retention of staff across the system – long- and short-term solutions such building affordable houses for health and social care staff were being considered; there was a focus on job and career prospects, to manage and support the acute care system, and to provide seven day care preventative services. Despite this support, staff in the hospital setting continued to report heavy workloads with additional pressures in relation to meeting performance targets. (Edited publisher abstract)
