It has been recommended that the special hospitals should be replaced by high-security services at a regional level. This study examines the implications of such a change in policy and demonstrates that local facilities would have to contain a significant proportion of patients whose violent and criminal behaviour can not be managed in medium security and where in some cases all previous therapeutic interventions have failed to prevent a progression through successively higher levels of hospital security over time. It provides little evidence that medium-security services reduced the demand for beds in special hospitals over the study period. The transfer of high-security beds to local services would have to take into account the threefold difference in levels of demand across regional catchment areas.

Quality of life (QOL) has gained importance as a global measure of social and clinical outcome in schizophrenia. This research aims to identify the clinical correlates of QOL at the time of first presentation with schizophrenia. Reducing the duration of untreated psychosis may have a beneficial effect on the subsequent QOL of patients presenting with schizophrenia.

Although mental health consumers often prefer community living, the factors influencing community outcomes are not well understood. In order to address this issue in Connecticut, the Department of Mental Health and Addiction services (DMHAS) commissioned a statewide study of 6,800 clients receiving the most intensive community-based mental health services funded by the state. Clients with better composite scores were those who had a prescription for medications, adhered more to their medication regimen, perceived to be less threatening, and had more frequent social interactions. Implications of these findings for outpatient treatment are discussed.
