This practice tool helps practitioners to collect information about a child's health and development and to analyse it in a logical sequence that identifies the type and seriousness of impairment to development. Weighed alongside other criteria, the results help to improve decisions about if and how to intervene in a child's life. The tool seeks to improve the consistency of practice decisions, both in relation to an individual practitioner's caseload and across children's services.
