Many studies have suggested that the presence of a child or adult with a disability in a family can have a significant impact upon family members, in particular the primary caregiver. Research findings have indicated that there may be social isolation, lack of awareness of services, poor service delivery and psychiatric disorders among parents. This article, part of a larger research undertaking, presents some key survey findings from a study of mothers and fathers of children and adults with cerebral palsy. The parents evaluated the caring process and their satisfaction with the professionals involved in service delivery. The prevalence of psychiatric morbidity among the parents is also reported. The findings indicate that the mental well-being of many fathers and even more mothers was severely affected by the caring process. High levels of parental dissatisfaction with some professionals are reported.

Examines psychiatric morbidity, using the General Health Questionnaire, which detects mainly anxiety and depression. The results of the follow-up data show that the most significant predictor of changes in psychiatric morbidity at follow-up was baseline psychiatric status, followed by health status. The strength of the study is its longitudinal design and verification of the results in three samples of elderly people, including one very elderly group. The lack of consistent associations with recovery from psychiatric morbidity (eg depression) in the literature enhances the importance of studies which aim to identify factors associated with different outcomes.

This article traces the development of home and community-based care to its current place in the worlds of health and social policy. An argument is developed to the effect that such services have by now gained both heightened policy legitimacy and organisational capacity. Building on these contentions, the articles goes on to suggest that such services should continue to gain a more prominent place within long-term care policy, and that long-term care issues deserve a more central place within social insurance policy more generally. The article concludes by suggesting that demonstrations of policy efficacy such as those that are taking place in home and community services might help to a least modestly offset the frontal assault which is currently taking place across the range of American social policy.
