Care of Europe's older people

This paper explores dignity from the viewpoint of older Europeans themselves. General recommendations concerning policies for older persons are considered together with commitments included in action plans on aging, in relation to the questions of to what extent these texts can be seen as a means to guarantee the dignity of the older person? The inter-relatedness of recommendations concerning health, labour market and those on social protection policies are stressed and the impact on the dignity of older citizens highlighted. The use of language is explored, as is the presentation of older persons in the media and advertising and the impact on dignity. The paper also questions whether dignity, or rather the loss of dignity, is experienced similarly by all groups of older persons. Is there a difference for men and women, for persons from ethnic minorities and for persons with a disability who are becoming old? Finally it considers what kind of care we want when we are old - how do we wish to be taken care of once we are no longer able to live fully independent lives in our own homes?

This study material was developed from an international research project 'Dignity and Older Europeans'. The material is derived from contributions from older people; health and social care professionals and other members of the public in the six countries which took part in the research UK (England and Wales), France, Ireland, Spain, Sweden and Slovakia. The learning activites are presented in four sections: Old Age - what does it mean?; Understanding Dignity; Dignity in care; and The impact of the system. Each section includes narratives based around the key themes that emerged from the study. The concluding section offers a brief discussion of the exercises and provides an overview of the main findings.

The Dignity and Older Europeans Research Group had produced a series of recommendations on policy. The policy recommendations are inspired by the data gathered from focus groups, as well as by the theoretical reflections on the concept of 'dignity'. This article introduces four core policy recommendations. The first reflects the rights of the older person, the second calls for the removal of ageism and ageist practices, the third concerns the regulatory frameworks needs in service provision and the fourth reflects the welfare entitlements of older people.
Aging with disabilities: ageism and more

Older people who also have disabilities find themselves in "double jeopardy" of experiencing prejudice and discrimination, which often lead to difficulty gaining access to needed healthcare and social services. This article examines that situation and suggests what must be done if those who are aging with disabilities are to be better served by the aging and disability service systems in the United States.

This paper describes the creation of the educational materials developed as part of the Dignity and Older Europeans Project. Following a discussion of the development process, the materials themselves are described. The materials include a poster of the dignity balance, which contains five core messages and illustrates the impact of both enhancing and violating individual dignity. The second product is a leaflet that also includes the dignity balance and lists of actions and approaches that will promote dignity or result in indignity. The final product to date is a multidisciplinary workbook, which is described in some detail. The workbook Educating for Dignity provides a brief outline of the theoretical model of dignity, and four different sections based on the empirical findings: understanding dignity, old age - what is it like to be an older person, dignity in care, and the impact of the system.

The role of professional education in promoting the dignity of older people

This paper considers the education of social and health care professionals who work with and care for older people. It asks whether education can promote the dignity of older people, how this may be done, what factors may ease or impede the promotion of dignity within professional education, and what part eduation plays alongside other influences on care practices. Beginning with consideration of research on the nature of professional education, the paper reviews principles of professional education, cultures and methods of teaching and learning and processes of practical apprenticeship. The paper argues that there are a number of challenges to the promotion of dignity within professional education, for example, inconsistencies in development of professional values, curriculum contradictions such as those between education for management and for direct care of older people, the balance between theory and practice and education for practice under changing real-world conditions.
Dependence, independence or inter-dependence? Revisiting the concepts of 'care' and 'dependency'

Research and theory on ‘dependency’ and ‘care-giving’ have to date proceeded along largely separate lines, with little sense that they are exploring and explaining different aspects of the same phenomenon. Research on ‘care’, initially linked to feminism during the early 1980s, has revealed and exposed to public gaze what was hitherto assumed to be a ‘natural’ female activity. Conversely, disability activists and writers who have promoted a social model of disability have seen the language of and the policy focus upon ‘care’ as oppressive and objectifying. ‘Dependency’ is an equally contested concept: sociologists have scrutinised the social construction of dependency; politicians have ascribed negative connotations of passivity; while medical and social policy discourse employs the term in a positivist sense as a measure of physical need for professional intervention. Autonomy and independence, in contrast, are promoted as universal and largely unproblematic goals. These contrasting perspectives have led social theory, research and policies to separate and segregate the worlds of ‘carers’ from those for whom they ‘care’. Drawing on the work of Kittay and others, this paper explores the ways in which sociological perspectives can develop new understanding of the social contexts of ‘care’ and ‘dependence’.
