This article connects and juxtaposes two important contemporary discourses in social policy: those of globalisation and of the 'welfare users' movements, particularly the movements of disabled people. It questions the widespread use of the globalisation process as a rationale for accepting that social policies should be subordinated to narrow economic constraints. It discusses the implications of social policies associated with globalisation for disabled people and other welfare service users, and their response to these. In particular the movements' emphasis on asserting choice in policy and their ability to influence the policy process is noted. The article concludes that there is much to be gained from a closer dialogue between the users' movements and social policy as a discipline, although there still seems to be some resistance to this from the latter.

The main part of the paper focuses on the extension of the Community Care (Direct Payments Act) to older people, drawing on research carried out by PSI and others, the recent practice guide on local authority implementation of direct payments produced by PSI and NCIL, as well as as NCIL's development work on IL and older people. It then considers the specific issues of : interpreting key direct payment criteria such as consent and 'willing and able'; models of assessment compatible with IL principles and putting in place appropriate forms of advice and support to enable older people to manage their own support arrangements.

Patient satisfaction is an important outcome measure of psychiatric care and a correlate of appropriate use of services. This research assessed out-patient satisfaction by questionnaire and surveyed environment quality at two clinic sites. High satisfaction was found in most areas of enquiry. Lower satisfaction was expressed with choice of available treatment, medication side effects and availability of information. Rapid turnover of doctors produced dissatisfaction with the consistency of medical care. Many patients wanted greater advocacy support. Facilities for the disabled could be improved. The best predictors of lower satisfaction were wishing for help to put one's views across and wish for more psychological help. Most psychiatric out patients were satisfied with their care. Dissatisfied patients may avoid out patients follow up and related surveys. Simple measures such as better provision of information and more explanation about medication side effects could increase satisfaction with services.
