Goal Attainment Scaling (GAS), an individualised measurement approach, is particularly attractive for the evaluation and care planning of frail elders, who often experience multiple, complex problems. Some service providers find GAS too unwieldy for routine use. A modified version of GAS that uses a standardized menu of goals and attainment levels has been developed by a regional referral hospital in Nova Scotia, Canada. This article reviews the development, implementation, benefits and challenges of the standardised menu, as well as the results of a retrospective descriptive study of its measurement properties.

Frail older people have often had to choose between residential care and staying in their own homes with community care services. The development of very sheltered housing in Wolverhampton has shown that there is a better way to meet the needs of frail and disabled older people.
Mental and physical frailty in older people: the costs and benefits of informal care

Describes the financial, opportunity and social costs and benefits of providing informal support to frail older people within an economic framework. Around half the supporters reported financial costs or lost social opportunities. A minority of supporters had reduced their working hours or withdrawn from employment because of caregiving. Nearly all supporters reported at least one social cost and identified at least one positive aspect of caregiving. The benefits of caregiving were not consistently related to co-residency, relationship of supporter or frailty type. Possible strategies for decreasing the costs and increasing the benefits of caregiving are discussed.

Over half a million older people currently live in long-term residential or nursing home care in the UK. Describes a qualitative study by Age Concern Institute of Gerontology King's College London which examines the consequences for adult children and spouses following an older person's admission to a care home. It focuses specifically on how and why a care home was chosen, any financial consequences for family members and their involvement in care-giving activities in a residential setting.

Presents a description of the development, implementation, and evaluation of the three year project CARE (Community Action to Reach the Elderly) in the USA. Evaluation of the process and the outcomes was conducted through interviews with administrators, volunteers, and clients of the project. Several themes emerged from these interviews which illustrate the success of the project. Project CARE serves as a model for other communities who have difficulties reaching frail elders in minority neighbourhoods.
