The National End of Life Care Programme was launched in November 2004, and has been instrumental in delivering the Department of Health's (2008) End-of-Life Care Strategy and the National Institute of Health and Clinical Excellence's (2011) Quality Standard for End of Life Care for Adults. National clinical audit of falls and bone health in older people

The first national clinical audit to investigate the care received by patients who have fallen and fractured bones (hip, wrist, arm, pelvis or spine) shows that an inadequate service is being provided by most local health services, and that there are unacceptable variations of care across PCTs and Trusts in England, Northern Ireland and Wales. The audit, commissioned by the Healthcare Commission and carried out by the Royal College Of Physicians' Clinical Effectiveness and Evaluation Unit (CEEu), shows that most PCTs and Trusts were nowhere near meeting national standards and guidelines from NICE, SIGN and the National Service Framework for Older People on the care and prevention of falls

Dignity and nutrition inspection programme: national overview

This report summarises the results from a national inspection programme of NHS hospitals in England. Inspects focused on two outcomes: looking at whether older people are treated with respect and whether they get food and drink that meets their needs. The inspections took place between March and June 2011 and used teams made up of CQC inspectors, a practising and experienced nurse and an ‘expert by experience’ (someone with experience of caring or receiving care). One hundred NHS hospitals were inspected and all the inspections were unannounced. Of those inspected: 45 hospitals met both standards; 35 met both standards but needed to improve in one or both; 20 hospitals did not meet one or both standards. Problems identified included: patients’ privacy not being respected; call bells being put out of patients’ reach, or not answered soon enough; staff speaking to patients in a dismissive or disrespectful way; patients not being given the help they needed to eat; and patients being interrupted during meals and having to leave their food unfinished. Appendices include details on what the Care Quality Commission looks for in an inspection and a list of hospitals inspected. Individual hospital reports giving findings at the time of each visit are also available from the Care Quality Commission.

Traditionally ‘intermediate care’ has often been used to refer to a range of services at the boundary of primary and secondary care, although there have been differing assumptions as to the goal of intermediate care, the intensity of support provision and the appropriate target groups. Confusingly, intermediate care has also been used within the hospital as a term for units located between the intensive care unit and the general ward. The current form of intermediate care provision started to shape up with the NHS Plan. This proposed a range of intermediate care services designed to bridge between hospital and home and to: help people recover and regain independence more quickly; bring about swifter hospital discharge when people are ready to leave; and avoid unnecessary long-term care.
