This essay asserts that the long-term care system, as we know it, has grown in response to public policy initiatives in other areas. This system is lacking in clear, coherent conceptual underpinnings. If significant culture change is to occur, societal values, public policy, and provider behaviour. must be responsive to the clear preference of consumers for services that respect their dignity by balancing the system toward home and community-based services and home-like residential care when needed.
