Based on case management experience with frail elderly clients, the authors point out how effective case management requires the use of clinical understanding and skills. To successfully link clients to services, psychosocial barriers to the acceptance of services must be addressed. The losses and dependency of old age stir up vulnerabilities and defences which may interfere with the client's support system and the use of services. Using case examples, the authors illustrate how clinical skills are helpful in dealing with clients and families around the painful and difficult issues of old age and the acceptance of dependency and services. Social workers are urged to respect the sophisticated and skilled work of case management.

Social services are often criticised for not consulting Asian elders on community care. Reports on an innovative project in Hounslow which offers Asian elders the opportunity to gain an understanding of the basic concept of community care and enable them to find out what it has to offer. For example, a course was run to introduce them to social services, domiciliary care, care in the home, residential care, and the notion of carers and carer support. Some Asian elders also visited sheltered accommodation, day centres and residential homes.

Argues that human service agencies fail to recognize the large contribution which older people make to their own care. Managers who understand that service users are co-producers of care and recognize the need to manage the users, as well as their own staff and relations with other agencies, are more likely to be able to deliver good services. In future a greater understanding of co-production and the development of theory and practice which will combine the management of co-production with the empowerment of users within the public sector will be essential.
