User participation policies in Norway and England – the case of older people and social care

User participation has become one of the most important concepts in the social care sector in many European countries, but the literature has mostly paid attention to disabled people or those with mental health problems. This article compares the user participation policies directed at social care for older people in Norway and England. Using a discourse analytical approach, a selection primarily of White papers from the 1960s until today are analysed. The analysis draws on the literature's discourse discussion, including a democratic/rights based discourse (full citizenship), a consumer discourse (consumers’ rights to choose welfare services), a co-production discourse (users and state/local authorities partnerships), and nuances of these discourses. The analysis shows that, while both countries start with variations of a democratic discourse, Norway develops a temporary and weak consumer discourse in a middle phase, then moves to co-production in current times. England, on the other hand, develops a comprehensive consumer discourse but also a surprisingly strong co-production discourse – the idea of a ‘Big Society’ – in early and current times. Exploring the meaning of a new assistive technology device for older individuals

Researching the outcomes of assistive technology devices (ATDs) for older clients is important to facilitate clinical decision-making. However, to understand the outcomes associated with ATDs, one must investigate the users’ experiences and acknowledge the user as an active participant in diverse social contexts. Purpose: To enhance understanding of the users’ perspective regarding ATDs, this study aimed to investigate the meaning of the ATD for older individuals still living in their home environment. Methods: To provide descriptions of ATD experiences, older individuals who received a new ATD to compensate for their challenges in moving around, assist in self-care or both were recruited for the study. Participants were interviewed twice, with a few months between interviews, about their experience in using their new ATD. The interview transcripts were analysed in a hermeneutical–phenomenological research approach. Results: The analysis revealed three recurring themes associated with the description of ATD experiences: “enabling performance and choice”, “transformation from requiring assistance to assisting others”, and “preparing for the future”. Conclusion: The results show that ATDs are used to enhance competence, mastery, control, self-worth, hope, and preparedness. The ATD service delivery should be client-centredd and the client should be acknowledged as an active participant in producing change. Implications for Rehabilitation: researching outcomes of assistive technology devices are complicated and should involve the user experience; assistive technology devices does not have to be used in concrete actions to involve meaning for the user; health care professionals should be sensitive towards the meaning dimensions of the assistive technology devices as experienced by the user throughout the service delivery process. EFORTT: ethical frameworks for telecare technologies for older people at home: final research report

EFORTT (Ethical Frameworks for Telecare technologies for older people at home) is an EU funded project which aimed to examine the ethical, social and gender implications of telecare interventions (remote care technologies worn, installed or embedded in homes) for older citizens in Spain, England, Norway and the Netherlands. The project involved the use of fieldwork (including documentary analysis, practice observations, and interviews with users, carers and practitioners) to gather information about preventive and responsive remote care practices, and recruitment of citizens' panels of older people and carers in each of the 4 partner countries as forums for views on telecare and future directions of care. Analysis of findings from the research was used to identify key themes and issues to inform discussion at the September 2010 Ageing with Technologies participative conference on care in Europe for practitioners, users, carers, policy makers and academics, the concluding session of which brought together proposals and priorities for future action on telecare which emerged from the discussions. This report describes the context and main findings of the research and its implications for policy and practice. The recommendations for ethical telecare development include providing care at home that is meaningful, sufficient and dignified, and ensuring engagement of older people in designing, developing and decision making about care.
