The outcomes of an inpatient treatment program for geriatric patients with dementia and dysfunctional behaviors

Evaluates the outcomes of an inpatient programme in the USA, designed to reduce severe agitated behavior in geriatric patients with dementia who could not be successfully treated on an outpatient basis. An individualized treatment plan was created for each patient that involved pharmacological and nonpharmacological interventions with behavioural, environmental, and psychological components. Significant improvements on these assessments were observed. Concludes that the longitudinal, multidisciplinary approach used in this study was effective in significantly reducing intrusive and dangerous behaviours while preserving or enhancing patients' cognitive and functional abilities.

Advance directives (ADs) are increasingly used and are seen as a way of directing care after the loss of mental capacity. There are, however, major potential problems associated with their use. They may require the withholding of treatment that will alleviate suffering, and have the potential to cause patients to lose abilities and live with unnecessary suffering or handicap. The requirements for a valid AD are discussed and a simple model for doctors to assess the validity of ADs is proposed.

Discusses how depression is a common condition among elderly patients undergoing a crisis-generating event such as stroke or hip fracture. The purpose of this study was to determine by means of the GDS the prevalence of symptoms of depression in these patients, to assess the specific importance of each of the factors potentially affecting the GDS score, and to analyses the change in the GDS over the course of the rehabilitation process and its association with corresponding changes in the patient's functional state.

The question of disclosure of dementia diagnosis has become of greater significance in recent years, probably in part due to people presenting at earlier stages of the disorder and the necessity for patients to be able to consent to drug treatment for Alzheimer' disease and greater public awareness of the condition. This article describes the effects on three people of learning their diagnosis, and a psychological intervention used with them. The methodological problems of research in this area are discussed, and the related question of what types of intervention may be used to maintain self-esteem, social functioning and cognitive abilities following diagnosis.
