Services for older people affected by HIV and AIDS are thin on the ground. The Republic of Korea (South Korea) and Japan are highly industralised and modern nations which are both influenced by the Confucian tradition of respect for elderly and family responsibility for the care of aging parents. In both countries the proportion of the elderly population is increasing. Japan, since the end of World War II, has utilized its government bureaucracy to help develop the social welfare system and to formulate social policies and programs for the elderly. Japan's tradition of samurai Confucianism is congruent with the commitment of the Japanese government to such social development as a matter of national policy. The Republic of Korea has not assigned a comprehensive planning role to its government bureaucracy. Lacking the mix of industrial/post-industrial infrastructure of Japan and not yet faced with the immediacy of a very large elderly population, the Republic of Korea's government has developed its social policies for the elderly in a more incremental manner, usually emphasizing small scale and piecemeal initiatives. With respect to social support, it has emphasized voluntary family efforts as congruent with the Korean (and Chinese) variant of Confucianism. This paper will compare and contrast these different approaches.

Late in 1993 the body of an elderly man living alone was found in a council flat in Brent, London. At the council's request an inquiry was carried out by Wally Harbert, a director of local social services for 20 years and currently director of planning and development at Help the Aged. His article goes beyond Brent and the particular death to reflect more widely on the needs of isolated elderly people and on the possible implications for policy.

There is now an increasing overlap in the characteristics and needs of the different populations in long-term hospital care, in nursing home care and in residential care. With the reduction of long stay hospital care there is increasing pressure to discharge patients either into nursing homes or residential care. The article seeks to distinguish between the care provided in nursing homes and in residential homes for older people.

It is often perceived that the needs of carers are adequately met at the time of and following the cared-for person's admission to a nursing home. Although the placement of an elderly person in a nursing home relieves the carer of the physical burden of caring, certain problems or stressors are still faced by carers. It was found in this study that many carers who had admitted the elderly cared-for person to the nursing at Queen Victoria Memorial Hospital in Australia had cognitive (knowledge) and effective (emotional) needs at the time of and subsequent to the elderly person's institutionalisation. A small proportion of the carers also had social needs following the post-placement experience.
