An outline of the Fit for Life programme, designed to help bridge the gap between NHS mental health services and main-stream community exercise groups with the aim of: increasing physical activity levels, reducing risk of falls and maintaining/improving mental health and well-being for older people with mental health conditions. The programme has evolved in the last four years to encompass the principles of recovery orientated practice whereby individuals are able to actively build a meaningful life while continuing to experience mental health problems or following a period of poor mental health. It consists of a 12 week community-based group, run three times a year. The format includes strengthening/balance exercises and Tai Chi. Case studies are included to illustrate impact and outcomes of the programme. (Edited publisher abstract)
Developing an intermediate care unit for older people with mental and physical illness

A 12-bed intermediate care rehabilitation unit for older people recovering from physical illness who have health problems is described. The Unit is in a 72 bed care home. It is nurse-led and also includes a physiotherapist, occupational therapist and senior mental health nurse in the team. promotes. Its ethos is to encourage independence and allow patients to achieve their objectives.

Home treatment provides brief, intensive care at home as an alternative to hospital admission. This article describes the work of the older people's home treatment team based at Penn Hospital at Wolverhampton, winners of the NIMHE Positive Practice Award for innovative commissioning. Their model reduces the risk of dependency and also helps older people re-engage with their families and communities.

Carers are often too wrapped up in their duties to think about their needs. This article reports on a pilot project in Kensington and Chelsea, run by charity wpf Counselling and Psychotherapy and funded by the council. The project aimed to offer carers of older people and people with mental health problems counselling to help them gain independence. The article also contains a brief case study.

Camden is one of 29 Partnerships for Older People (POPP) pilot sites which have received government money to test out 245 projects to improve care of older people. This article looks at progress in Camden, the types of projects they are running under the 'Community Interventions for Older People with Mental Health Needs' and the early findings from the national evaluation.

Substance abuse programs designed for the needs of older adults are rare in the United States, and traditional mental health providers are often reluctant to work with the population, thus making it difficult for the homeless older adult to receive mental health services. This article looks at the work of the Saint Mary's Center in California which obtained funding for three years to provide services for elderly homeless people with mental health problems.

Community mental health teams (CMHTs) for older adults inevitably encounter service users with alcohol use disorders (AUDs). However, little attention has been given to the needs of older adults with a dual diagnosis. This paper argues against a wholesale transfer of knowledge and practices from working age adults with dual diagnosis to the older age group. Drawing on the experience of care professionals in a central London’s CMHT between 2008 and 2010, the paper illustrates the challenges encountered when working with older adults with co-morbid mental health problems and AUDs. The management and progress of 4 older adult service users are described. This case study series illuminates some of the clinical and professional dilemmas and challenges encountered when working with this group. Suggestions of how to respond to some of these challenges are provided by focussing on aspects of identification, assessment and treatment.

The author describes the opening of a pub in a residential home for older people with mental health needs. It was part of a larger project which consisted of a series of artist led-workshops with residents to create art work; sourcing and purchasing brewery memorabilia for display in the pub, and constructing the pub itself. It has now transformed a previously unused seating area into a valuable resource.
