The Care Programme Approach (CPA) should be applied to older people with severe mental illness due to schizophrenia or other psychoses. The assessment of their needs should be based on the Single Assessment Process (SAP) for older people. SAP, plus critical aspects of CPA, should be applied to other older people with severe functional or organic mental health problems, who were they younger would be provided for under CPA. When individuals subject to CPA reach old age, switches to SAP are not inevitable.

While broad in scope, these standards acknowledge the unique and complex needs of individuals, and the additional specific knowledge, and skills required in order to deliver a service that is tailored to the needs of each person. With the emphasis on caring for people with complex health and personal care needs living in their own home instead of in residential or nursing homes or long stay hospitals, the provision of personal domiciliary care services is evolving rapidly and reflects changes at the interface between health and social care. These standards will be applied to agencies providing personal care to the wide range of people who need care and support whilst living in their own home, including: older people, people with physical disabilities, people with sensory loss including dual sensory impairment, people with mental health problems, people with learning disabilities, children and their families, and personal or family carers.

This is the second annual report of the Health and Social Care Change Agent Team (CAT). It sets out the work carried out by members of the team, CAT's associates and expert group members in 2003/4. It is meant to be a useful source of practical information and examples of ‘what works’, not just a historical account of past activity. In summary, in 2003/4 CAT has: worked with a further eight health and social care communities, involving a total of 15 councils, 29 PCTs, 20 hospital trusts and 6 other trusts; delivered a dedicated programme to support the introduction of reimbursement under the Community Care (Delayed Discharges etc) Act 2003; produced a further range of development and support products on commissioning and other good practice themes; begun to plan and implement new work on supporting improvements in mental health services for older people (jointly with the National Institute for Mental Health in England) and establishing a new learning and improvement network on better commissioning; and contributed to 67 national and regional events, and directly organised 24 events.
