The Forth Valley inspection covered a wide range of services for older people - mainly health and social work. It focused on NHS Forth Valley, Clackmannanshire, Falkirk and Stirling Councils. It looked at the way that these services worked together to assist older people live as independently as possible. This leaflet summarises the key findings of the inspection. Examples of good services and positive collaborative working are presented.

One of 20 reviews of local authority areas which look at how older people move through the health and social care system, with a focus on how services work together. The review focuses on three key areas: maintaining the wellbeing of a person at home or in their usual place of residence; crisis management; and step down services and returning a person to their normal place of residence. The review found that although Cumbria have a health and wellbeing strategy and a vision for the whole of Cumbria, there was inconsistency in how the strategy was interpreted and how services were delivered across the two different sustainability and transformation partnerships (STP) areas. Although staff were not always able to articulate a system wide vision, reviewers identified examples of staff working in an integrated way to improve outcomes for people. People’s experiences of services depended on the type of service they received. Some people experienced delays in hospital discharge due to delays in the assessment process, lack of home care packages and a shortage of care home placements. Barriers to integration also included separate ICT systems that led to difficulties in information sharing and duplication of effort; and a lack of suitably skilled health and social care workforce. Suggested areas for improvement include the development of implementation plans to deliver a county wide strategy; the development a coherent health and social care workforce strategy; and for a review reablement provision and services across Cumbria. Orchid View: serious case review

This independent serious case review was commissioned to investigate safeguarding concerns that occurred within Orchid View care home before its closure in October 2011. Orchid View provided care with nursing for up to 87 older people and people with dementia from September 2009 until it was closed in October 2011 due to safeguarding concerns. The case review was instigated in August 2013, once the police investigations and safeguarding investigations were completed. The review also address questions raised by the families of residents at the home and considers the broader context in which care was provided and the impact of these issues on other settings in the future. Findings and recommendations are reported in the following areas: safeguarding concerns and actions; Orchid View's development and opening; financial and governance arrangements of care providers; Care Quality Commission's; safeguarding awareness and agencies working together;information for residents and relatives; workforce issues; and accountability. The review identifies an avoidance of positive action to rectify problems, and a series of ineffectual action plans that were not acted on. (Original abstract)
