A five-year strategic plan setting out how the Nuffield Trust will work to improve health care in the UK. The plan centres around a renewed focus on what policies mean on the ground and on bringing together policy-makers with frontline clinicians and managers. The trust is committed to: improving the evidence base that leads to better care for people in the UK through our research and analysis; using its independence to provide expert commentary, analysis and scrutiny of policy and practice; and bringing policy-makers and NHS staff together to raise issues and identify solutions. Work will focus on five key areas, selected for the opportunities to contribute to solving problems in the coming years: quality of care - new models of health care delivery; workforce; older people and complex care; and providing independent scrutiny of government policies and the performance of the system. UK healthcare: development opportunities 2014

An analysis of current trends in the demand for new care home development in the UK. It shows that demand is expected to improve going forward, driven by strong growth prospects in the UK's population of over 65s and as current stock approaches obsolescence. The analysis reveals that more than half of the 12 counties in England and Wales with the highest prospects of development are located in the UK's southern regions, with Greater London climbing to the top spot. The Scotland Hotspots analysis reveals that Highlands and Islands has replaced Borders as the top ranked county for care home development. (Edited publisher abstract)
Predictive validity of tools used to assess the risk of unplanned admissions: a rapid review of the evidence

A synthesis of evidence assessing the predictive ability of tools used to identify frail elderly and people living with multiple long-term chronic health conditions who are at risk of future unplanned hospital admissions. There are now a large number of models available that can be used to predict the risk of unplanned hospital admissions and this study aims to provide a summary of their comparative performance. Overall, the models identified in this review show reasonable concordance in terms of their predictive performance (based on c-statistics). Models reporting other performance indications showed that at different thresholds, as sensitivity increased, specificity would decrease. As the algorithms become more complex or incorporate longer term horizons specificity increased but the ability of the models to identify future high cost individuals reduced. It should also be noted that whilst the reported c-statistics are broadly similar, the underlying populations, data sources and coding may differ. (Edited publisher abstract)

This commentary considers the demographic 'bulge' of the so-called 'baby boomer' generation and the public mental health challenge that this group will represent as they move into old age. It is argued that now is the time to start a debate with the aim of identifying long-term solutions to these new challenges.

Increasing the current service provision in line with the projected population growth is unaffordable and early evidence from the Anticipating Future Needs studies shows that it would not meet the changing expectations of older people. Configuring Future Services explores service configuration options to support people's preferred lifestyle, in their own homes, as long as possible. These exist at the health and social care interface and represent opportunities to build on Joint Strategic Needs Assessment with specific, practical joint projects. This paper provides the evidence for service reconfiguration for 7 conditions based on the current literature and an implementation planning approach. A second paper, in December 2007, will discuss lessons learnt from work with two pilot authorities and make recommendations for wider implementation including the building of a knowledge base for local authorities to access.

Supporting a group of very frail people with dementia to relocate is a complex task. In this article, the authors describe how they planned and implemented an internal relocation at Amerbly Lodge in London in order to minimise the risks to residents, and also the important lessons learned.

This guide is part of the process of disseminating the findings of the research study, Implementing the National Service Framework for Older People: looking at older people and carer involvement. Overall, it represents a synthesis of best practice on the involvement of older people and carers in the strategic planning processes which inform the provision of health and social care. It is derived from the views of older people and carers on the practicalities of involvement, research findings and relevant literature. The title, Clear Voices, was chosen by older people who participated in the study, emphasising the importance they attach to their voice being represented in the strategic planning process. In the first instance, the aim of the guide is to assist Local Implementation Teams and representative older people and carers to work together, although the principles on which it is based have a wider applicability. It provides a framework for agreeing a joint vision for the future, auditing current practice regarding lay involvement in decision-making, identifying the barriers to progress and deciding together how to move forward. The guide is in two parts. Part One identifies the prerequisites for successful change, specifies standards for best practice, summarises key findings from the research and describes a process for continuous improvement. Part Two comprises practice tools which provide detailed guidance on how to audit and develop the key processes integral to the involvement of older people and carers in the health and social care strategic planning process.
