Older people often require a range of services from different sources. Integrated care offers a co-ordinated approach to meet their often complex needs. This working paper acts as a checklist for national and regional policy-makers across Europe to use in drawing up their own integrated care policies. It can also be used by service users, their carers, and older people’s organisations, to assess the policies in place in their countries or regions. The publication was funded by the European Commission, and is one of several publications produced by the Care and Management of Services for Older People in Europe Network (CARMEN), which is run by the European Health Management Association (EHMA).

This guide is part of the process of disseminating the findings of the research study, Implementing the National Service Framework for Older People: looking at older people and carer involvement. Overall, it represents a synthesis of best practice on the involvement of older people and carers in the strategic planning processes which inform the provision of health and social care. It is derived from the views of older people and carers on the practicalities of involvement, research findings and relevant literature. The title, Clear Voices, was chosen by older people who participated in the study, emphasising the importance they attach to their voice being represented in the strategic planning process. In the first instance, the aim of the guide is to assist Local Implementation Teams and representative older people and carers to work together, although the principles on which it is based have a wider applicability. It provides a framework for agreeing a joint vision for the future, auditing current practice regarding lay involvement in decision-making, identifying the barriers to progress and deciding together how to move forward. The guide is in two parts. Part One identifies the prerequisites for successful change, specifies standards for best practice, summarises key findings from the research and describes a process for continuous improvement. Part Two comprises practice tools which provide detailed guidance on how to audit and develop the key processes integral to the involvement of older people and carers in the health and social care strategic planning process.

While this one-day conference was based on a number of different individual presentations and on a series of parallel sessions, several key themes emerged from the day which may have implications for future policy and practice. They were: partnerships as a means to an end; person-centred services; avoiding structural solutions; and seeking local solutions.
Enhanced skills training and health care management for older persons with severe mental illness

This report describes a combined skills training (ST) and health management (HM) intervention for older adults with severe mental illness (SMI) and one-year pilot study outcomes. Findings are reported for twelve older persons with SMI (age 60+) who received ST+HM and twelve who received only HM. ST addressed interpersonal and independent living skills. HM included promotion of preventive health care. ST+HM was associated with improved social functioning and independent living skills, whereas functioning remained constant or declined for the HM only group. Both groups receiving HM demonstrated increased use of preventive health services and identification of previously undetected medical disorders.
The development of a short instrument to identify common unmet needs in older people in general practice

No structured needs assessment tool appropriate for older people and also suitable for use in routine general practice consultations exists. This study aimed to engage older people in the development of a brief, valid, practical and acceptable instrument to help identify common unmet needs suitable for use in routine clinical practice in primary care by user involvement in a multi-stages approach to heuristic development in general practices, voluntary groups and community organisations in north and central London. Subjects included patients 65 and over in purposively selected practices, voluntary organisations for older people, community organisations involving older people, GPs and community nurses. Data were collected through mixed methodology interviews using a structured assessment tool, a postal questionnaire, and focus groups. Synthesis and interpretation was done through a consensus conference followed by a Delphi process involving primary care professionals. Five domains of unmet need were identified as priority areas by all 3 methods, the consensus conference, and the Delphi process: senses (vision and hearing), physical ability (mobility and falls), incontinence, cognition, and emotional distress (depression and anxiety). Concludes that public involvement in the design of clinical tools allowed the development of a brief assessment instrument that could potentially identify common, important and tractable unmet needs in older people.
Scarcity of HIV/AIDS risk-reduction materials targeting the needs of older adults among state Departments of Public Health

This US study investigated the availability of printed human immunodeficiency virus/acquired immune deficiency syndrome (HIV/AIDS) education/prevention materials from state departments of public health within the United States, which specifically targeted the older adult population. Information on HIV/AIDS from public health departments in each of the 50 states was solicited, obtained, and analyzed for applicability to an older adult population. Although all 50 states responded to our request for publications, only 15 states (30%) provided publications that were specifically intended for an older adult audience. The title, date of publication, and publisher of publications that were considered "specific to older adults" are provided. These findings suggest that there is a general lack of HIV/AIDS prevention literature specifically tailored for older adults available through state departments of public health.

Considers the impact of reimbursement on delayed transfers and whole-systems planning. Argues that making hospital discharge work with the focus only on the interface between health and social care will have limited and short-term results. More radical cultural change is needed to tackle the problems of an ageing society. Draws on recent thinking which challenges the way in which services are currently provided to older people and calls for new ways for services to be commissioned and provided. Advocates changes towards community-based processes and pathways across a wide range of services and within a much broader whole-systems framework, to support independence.
