Agreement between dementia patient report and proxy reports using the Nottingham Health Profile

The aim of the study was to examine the agreement between patient reports and their proxy reports (family and care provider proxies) on Health Status in a sample of patients with dementia. Completion rates for the different NHP dimensions ranged from 78 to 90% for the dementia subjects. Inter-rater agreement between different proxies and subject was from moderate to good for physical assessment (ICCs from 0.54 to 0.78 for physical mobility scales). Patient/family proxy concordance was moderate to good for five out of six dimensions (physical mobility, social isolation, pain, energy, sleep) and poor for emotional reaction. Family proxies systematically reported lower functioning than did patients in the four subscales assessing: physical mobility (p < 0.0001), energy (p < 0.005), social isolation (p < 0.01) and sleep (p < 0.03). Care provider proxies only estimated physical mobility as lower (p < 0.0001). Age and physical status of the patient significantly affected agreement in patient-care provider proxy ratings. Thus, caution is appropriate when resorting to proxies to estimate the Health Status of a dementia patient.

This article describes the work of REACH (Resources for Enhancing Alzheimer's Caregiver Health), a multi-site research programme which carries out social and behavioural research on interventions designed to enhance family caregiving for Alzheimer's disease and related disorders. The research aims to test the effectiveness of multiple interventions and to evaluate the overall outcomes of such interventions.

Well being, or quality of life, is a continuing goal for individuals and a major criterion for the evaluation of governments and societies. As a research concept, however, it has been marked by persisting problems of definition and measurement and by uncertainties about its changing pattern over the life course. In this article, these issues are discussed; the concept of well–being is “unpacked” and a model of stocks and flows is described as applicable to the analysis of well–being. Finally, the concept of resilience is proposed as important for research on well–being in relation to age.

This article explains a new concept, positive spirituality, and offer evidence that links positive spirituality with health. It describes effective partnerships between health professionals and religious communities and summarizes the information as a basis for strengthening the existing successful aging model proposed by Rowe and Kahn. A missing component to Rowe and Kahn's three-factor model of successful aging is identified, and we propose strengthening the model with a fourth factor, positive spirituality. Evidence presented suggests that the addition of spirituality to interventions focused on health promotion has been received positively by older adults.

Health care for elderly South Asian people may not be designed to best meet their needs. This article describes the development of a service for South Asian elders in London, and reports the results of a study which revealed considerable unmet health and social service needs in this sector of the population.

The London Older People’s Service Development Programme was a two-year, two phase service improvement programme covering most of London between 2001-2003. Run by the London Directorate of Health and Social Care (LDHSC), it used the ‘collaborative’ methodology, successfully employed in other national service improvement programmes for primary care, cancer, coronary heart disease, medicines management, and more recently emergency care. A total of 25 Borough-based projects took part.

Community elders were interviewed by telephone twice at baseline using both computer-assisted telephone interviews (CATI) and nurse clinicians, and at four-month follow-up using CATI to collect alternatives paraprofessional home care adequacy measures. Findings support emerging approaches in primary care and home health quality measurement that consider multiple dimensions of user response to formal and informal care.
