The elderly population is increasing more than any other sector of the population. Dementia increases disability, morbidity and mortality among older people. For these reasons the possibility of predicting progression and prognosis has enormous importance. This article summarises the main findings from the Kungsholmen Project a longitudinal epidemiological study on dementia that started in Stockholm, Sweden. Aims to help give a better understanding of the natural history of dementing disorders and provide hypotheses for further research.

This study compares the validity of different instruments for the screening and diagnosis of dementia and to provide threshold scores for these purposes, i.e. screening focusing on a high sensitivity and diagnosis focusing on a high specificity. Concludes that there is no perfect instrument for the screening and diagnosis of dementia. Recommendations for improving the validity of the Delayed Word List Learning Task for discriminating dementia and cognitive health include the expansion of list length and shortening of delay.

The present study describes the development and validation of an informant interview for the diagnosis of dementia and depression in older adults (IDD-GMS), which is based upon the well established Geriatric Mental State Schedule (GMS). Found that the validity and reliability of the IDD-GMS falls within acceptable limits and indicates that the IDD-GMS can be used as a diagnostic instrument for dementia and depression. The IDD-GMS represents the first informant interview to achieve this.

This article suggests that the consensus view of psychometric assessment in mental health settings as a relatively bland, benevolent activity is misguided in a number of important aspects. Rather than being effectively neutral, it is suggested that the experience of assessment for older adults in fact arouses anxiety in the individual concerned. Highlights the need for improvement including: a greater questioning of the purpose of memory assessment by the professionals involved, and of its actual practice, in order to match the pace of assessment to that of the older person with cognitive impairment; a firmer grasp on the biographical context of the person's life history; and a sharing of control over the direction of the assessment.
