Community-based treatment of late life depression: an expert panel-informed literature review

This systematic review employed an expert panel to assess the quality of studies and make judgements on the effectiveness of those interventions for which sufficient data were available. Adequate data existed for depression care management, group and individual psychotherapy for depression, psychotherapy targeting mental health, psychotherapy for carers, education and skills training (to manage health problems besides depression, and for carers), geriatric health evaluation and management, exercise, and physical rehabilitation and occupational therapy. Depression care management was rated as effective; education and skills training, geriatric health evaluation and management, and physical rehabilitation and occupational therapy were rated as ineffective; and the remaining interventions received mixed ratings. Several interventions lacked sufficient data for assessment because of limited numbers of studies and/or poor study quality. The authors stress that some interventions found to be of mixed effectiveness (or even ineffective) may have a part to play in a multifaceted approach to the treatment of depression in later life.

Mental health status of home care elders in Michigan

This study describes the mental health status of community-living frail elders in Michigan and identifies subgroups of individuals who are vulnerable to mental health problems. The study analyzed the baseline assessment data collected from older adults who were admitted to two community-based long-term-care programs in Michigan (N = 18,939). The programmes aim to help adults at risk of nursing home placement to remain in the community by providing them with supportive services. Results show that 40.5% of the individuals in the sample have recognized mental disorders, 39.6% use psychotropic medications, 24.5% have probable depression, and 1.4% have self-injury thoughts or attempts. Frail elders who are White, younger, and female - as well as those who experience more pain, disease burden, cognitive impairment, and limitations in instrumental activities of daily living - are more prone to psychological distress. Mental health care is greatly needed by community-living frail elders.
The Olmstead decision and the journey toward integration: the evolution of social work responses

The 1999 Supreme Court decision on Olmstead v. L. C. declared that the medically unjustifiable institutionalisation of an individual with mental disabilities was discriminatory if that person could be appropriately treated in the community and had had sought such treatment. It triggered an expansion of community-based care for all people with disabilities, including older people, and reinforced an existing trend towards social integration. This paper traces the evolution of US perspectives on disability from the pre-1960s to the Olmstead decision, and the accompanying development of social work intervention models from case work, to case management, to consumer-directed care, to care coordination within an empowerment framework. The author notes that more traditional approaches such as case management still prevail and that social workers need to consider how their practice should respond to the evolving policy environment. As long-term care costs continue to risk, preventive, low cost community services to support older people are needed. This article examines the kinds of schemes that are required. The article highlights good practice in the areas of: third sector partnerships; a healthy eating and falls initiative from Age Concern; the use of time banks; and telephone trees which can be a useful community support network.
