Mentalization in dementia care: an autoethnographic account of a project worker’s experiences

Purpose: Mentalization is a psychodynamic concept that can help us to understand our emotional responses to others. The purpose of this paper to illustrate how the concept of mentalization may be applied in dementia care. Design/methodology/approach: An autoethnographic account of the author’s experiences (first author), working as a project manager in which the author used the concept of mentalization to pay close attention to how the author’s emotional responses to people with dementia influenced thier communicative interactions. Findings: This paper outlines how the author processed the author’s own internal experiences in both mentalizing and non-mentalizing modes, as the author wrestled with feelings of conscious incompetence. In the non-mentalizing mode, the author was pre-occupied with the author’s own anxieties. The author struggled to relate to or make sense of the experiences of the individuals with advanced dementia that the author engaged with. Moving towards a mentalizing stance helped the author to attune to the embodied experiences of the people with dementia and recognise the reciprocal nature of our communicative interactions. Originality/value: This paper illustrates the role that mentalization may play in developing natural and authentic strategies to support communicative engagement in dementia care. These strategies may be of potential value to family carers. Family carers who can maintain a mentalizing stance may be more able to respond in empathic, person- centred ways to people who are living with dementia. On the other hand, non-mentalizing responses may be a root cause of mis-understanding and emotional disengagement. (Publisher abstract)
Age-related hearing loss: quality of care for quality of life

Age-related hearing loss (ARHL) is a major public health problem, resulting in reduced speech understanding, for millions of geriatric patients. ARHL is the third most prevalent chronic condition in older Americans, after hypertension and arthritis, and is a leading cause of adult hearing handicaps in the United States. Between 25-40% of the population aged 65 years and older is hearing impaired, and the prevalence is expected to rise with the increasing ageing Baby Boomer population. Despite this, ARHL remains an often undetected, underestimated and neglected condition due to the slow development process of the disease. The consequences of ARHL (social isolation, reduced environmental stimulation and depression) may aggravate cognitive decline. Health care professionals who serve the geriatric population need to understand the fundamental ARHL-associated changes that affect auditory and cognitive processing of speech and aural communication, in order to improve the quality of hearing health care and the overall quality of life of the Baby Boomer generation. The purpose of this review is to raise the awareness of ARHL, to update understanding of ARHL with a focus on age-related deficits in auditory and cognitive processing of speech, and to explore strategies of prevention, identification, amplification, and aural rehabilitation.
The importance of hearing: a review of the literature on hearing loss for older people with learning disabilities

Background: Hearing loss has a significant impact on living well and on communication in all adults, with the numbers affected increasing with age, and adults with learning disabilities being at particular risk. Methods: A review of the literature on hearing loss in older adults with learning disabilities was completed. Results: A significant increase in hearing loss with increasing age in this group was demonstrated, at a greater level than in the general population. Prevalence rates have been the main focus of research, with few considerations of access to hearing assessment or benefits of rehabilitation such as hearing aids, or the effect that age has on living with hearing loss. Conclusions: With advances in audiology services and increased life expectancy of those with learning disabilities, further research would be valuable. In addition, all health and social care services should consider actively promoting hearing assessment and rehabilitation. Individuals, carers and families should also take action, not only when concerns around memory arise, but proactively for all. (Publisher abstract)
Effectiveness of the validation method in work satisfaction and motivation of nursing home care professionals: a literature review

The purpose of this study was to carry out a literature review on the effectiveness of the validation method (VM) in job satisfaction and motivation of care professionals working with older people in nursing homes. The review was carried out in specialised databases: Scopus, PsychINFO, PubMed, Web of Science (WOS), Google Scholar, Scielo, and Cochrane Database of Systematic Reviews. 9046 results were obtained, out of which a total of 14 studies met the inclusion criteria: five quantitative, four qualitative, one single case series, two quasi-experimental and two mixed methods studies. The results of the analysed studies report that the VM can be an effective tool that facilitates communication and interaction in care, reducing levels of stress and job dissatisfaction among care professionals. The VM facilitates communication between professionals and older people with dementia, and improves the management of complex situations that may arise in care, directly influencing a reduction in work stress and increasing job satisfaction. Experiences of elderly patients regarding participation in their hospital discharge: a qualitative metasummary

Background: Ageing patients are discharged from the hospital ‘quicker and sicker’ than before, and hospital discharge is a critical step in patient care. Older patients form a particularly vulnerable group due to multimorbidity and frailty. Patient participation in healthcare is influenced by government policy and an important part of quality improvement of care. There is need for greater insights into the complexity of patient participation for older patients in discharge processes based on aggregated knowledge. Objective: The aim of this study was to review reported evidence concerning the experiences of older patients aged 65 years and above regarding their participation in the hospital discharge process. Methods: The study conducted a qualitative metasummary. Systematic searches of Medline, Embase, Cinahl, PsycINFO and SocINDEX were conducted. Data from 18 studies were included, based on specific selection criteria. All studies explored older patients’ experience of participation during the discharge process in hospital, but varied when it came to type of discharge and diagnosis. The data were categorised into themes by using thematic analysis. Results: The analysis indicated that participation in the discharge process varied among elderly patients. Five themes were identified: (1) complexity of the patients state of health, (2) management and hospital routines, (3) the norm and preference of returning home, (4) challenges of mutual communication and asymmetric relationships and (5) the significance of networks. Conclusions: Collaboration between different levels in the health systems and user-friendly information between staff, patient and families are crucial. The complexity of patient participation for this patient group should be recognised to enhance user involvement during discharge from hospital. Interventions or follow-up studies of how healthcare professionals can improve their communication skills and address the tension between client-centred goals and organisational priorities are requested. Organisational structure may need to be restructured to ensure the participation of elderly patients. (Edited publisher abstract)
Improving practice in communication with older people and support networks living in housing with care schemes: aspirations and ambitions

In the UK, the options relating to housing with care will likely increase, as they promote choice and independence. If housing with care services offer end-of-life care to their service users, their staff will need to engage with social workers and other professional networks and these professionals will need to understand the context and ethos of housing with care provision. In order to investigate whether existing literature will assist professional communication across sectors, a scoping literature review was conducted. Findings were grouped into themes, according to the project brief, focusing on: confidence and skills, making decisions and plans, and access to support and training. Several elements were identified as potentially supportive of staff confidence, such as obtaining biographical information, mutually respectful working relationships, managerial support and better understanding of specialist communication needs.

This web-based briefing provides a concise summary of the research and policy literature into the means, benefits and difficulties of involving patients in the planning of discharge to community or intermediate care. It also considers the role of carers in this process, as well as what happens when an older person’s ability to communicate their preferences in these matters is affected by dementia, language difficulties, or an unwillingness or reluctance to express preferences about the provision of care. The briefing also examines policy and research findings on older people’s involvement in discharge planning more generally. The briefing was commissioned by the Social Care Institute for Excellence (SCIE).
Sensory loss and its consequences for couples’ psychosocial and relational wellbeing: an integrative review

Objectives: Previous research has shown that marital communication is key to couples' successful illness adjustment. However, little is known about couples' experiences of health conditions characterised by communication difficulties such as acquired hearing, vision, and dual-sensory loss. The aim of this review was to identify the effect of sensory loss, and associated communication difficulties, on couples' relational and psychosocial adjustment. Method: A systematic search was conducted to identify studies investigating the social, psychological, and relational impact of sensory loss on couples. Results: Twenty-four articles met the criteria for inclusion in the review. Significant heterogeneity in the measurements and design of the quantitative studies prevented statistical data synthesis. All but two studies reported some effect of sensory loss on couples' psychosocial or relational wellbeing. Higher levels of marital satisfaction were found to buffer against adverse psychological outcomes. Results of quantitative and qualitative studies were synthesised to form an integrative model illustrating the associations between sensory loss and couples' relational and psychosocial wellbeing. Conclusions: Although this review reports an association between sensory loss and couples' relational and psychosocial wellbeing, the results should be viewed with caution given that relatively few studies on couples' experiences of acquired sensory loss exist, and many have methodological limitations. Low health literacy and older adults: meanings, problems, and recommendations for social work

Many older adults struggle to manage their health care problems. Low health literacy exacerbates such struggles and contributes to a variety of adverse health behaviours and outcomes. Addressing how health literacy impinges on the lives of older adults is a neglected area of social work practice and knowledge. This article explores seven areas: defining health literacy, the problem and prevalence of low health literacy among older adults, health inequalities and health literacy, a brief literature review, neglected issues in the literature, suggestions for macro and micro social work interventions to improve health literacy for older adult populations, and conclusion.
