The reliability of the complex element of the Northern Ireland single assessment tool for the health and social care of older people

The Northern Ireland Single Assessment Tool (NISAT) was developed to address the assessment process as highlighted in the NSF for Older People. The NISAT consists of three main parts: the Contact Screening; the Core NISAT; and the Complex NISAT. This study involved examining the reliability of information collected using the complex assessment instrument of the NISAT. Two measures were calculated: the ability of assessors to identify the themes contained in case studies; and the total number of themes identified by each assessor to provide information on the comprehensiveness of the assessments. A summary of results by case study are presented. Key findings supported the reliability of the complex NISAT; identified variability in the assessors’ ability to identify the themes/measures; highlight areas for potential modification; and identified assessors desire for comprehensive training and clear guidance on the correct way to complete the Complex NISAT. The study confirmed the reliability of the Complex NISAT and the use of case studies in the development and testing of assessment tools, especially in the healthcare setting. Appendices include details of the three case studies used.

There are over 200 000 people in the UK diagnosed with Aspergers Syndrome (AS). Most of these are children and young adults, owing to the fact the disorder was established relatively recently. It can be argued, therefore, that there are many older adults who may have met the criteria for AS as children, but never received such a diagnosis due to the fact it had yet to be established. What happened to these people as they aged? This paper examines this issue in detail and presents five case studies of elderly individuals who the authors believe meet the criteria of AS. The work illustrates AS presentation in old age, the assessment problems and tools required to assess older people, and the implications of such formulations for clinical practice. Older patients with undiagnosed AS may currently be receiving inappropriate treatments. Greater awareness of AS in the older population would enable better management of such patients.

This is one of a suite of case studies designed to increase awareness of schemes to move healthcare closer to home. The Older People’s Assessment and Liaison (OPAL) Service at Gloucestershire Hospitals NHS Foundation Trust ensures that older people presenting at the trust’s A&E receive comprehensive clinical assessment and care planning from a consultant geriatrician. An important feature is the OPAL consultants’ close working with the integrated discharge team to facilitate supported discharge and recovery at home. (Edited publisher abstract)
