Addressing the needs of older people with co-morbid alcohol and mental health disorders: a case series from a London Community Mental Health Team (CMHT)

Community mental health teams (CMHTs) for older adults inevitably encounter service users with alcohol use disorders (AUDs). However, little attention has been given to the needs of older adults with a dual diagnosis. This paper argues against a wholesale transfer of knowledge and practices from working age adults with dual diagnosis to the older age group. Drawing on the experience of care professionals in a central London’s CMHT between 2008 and 2010, the paper illustrates the challenges encountered when working with older adults with co-morbid mental health problems and AUDs. The management and progress of 4 older adult service users are described. This case study series illuminates some of the clinical and professional dilemmas and challenges encountered when working with this group. Suggestions of how to respond to some of these challenges are provided by focussing on aspects of identification, assessment and treatment.
Never too late: older people and alcohol misuse

This paper explores some of the issues around older people and alcohol. The paper highlights good practice through case studies and identifies some practical ways to prevent and reduce the risk of alcohol abuse by older people. More brief intervention for older drinkers, more specialist projects for older drinkers, and training and support for staff in all services dealing with older people who misuse alcohol is urgently needed. Additionally, specialist training for dementia services to enable them to work effectively with alcohol-related brain injury and drinkers in drug and alcohol services who are ageing are also needed. Finally, the need more research on substance misuse in older people, primarily in order to generate practical responses is highlighted.

This compilation of case studies is intended to show how local authorities continue to make progress on improving health and wellbeing and tackling health inequalities since public health was formally transferred from the NHS in April 2013. Public health teams, working with a ‘Health in All Policies' approach across councils, are tackling persistent problems like adult and childhood obesity, mental illness, alcohol abuse, sexually transmitted infections and the health impact of isolation and loneliness in old age, as well as addressing some of the serious health inequalities that still exist within and between communities. These case studies aim to show what potential there is for public health, if properly resourced, to make inroads in improving health and wellbeing. The case study sites cover Birmingham City Council; Cambridgeshire County Council and Peterborough City Council; Cheshire and Merseyside Public Health Collaborative (Champs); London Borough of Redbridge; Redcar and Cleveland Council; Sheffield City Council; Somerset County Council; and London Borough of Tower Hamlets. Social workers provide older people with varied services in diverse settings and are in a position to assume leadership in slowing the spread of AIDS. The major risks among older people include sexual activity without contraception, alcohol and drug abuse, blood transfusions before 1985, and misdiagnosed diseases such as Alzheimer's, Parkinsons, respiratory disease and sexually transmitted diseases. This American article discusses guidelines that can help social workers provide prevention education to older people.

DrugScope has produced this briefing on behalf of the Recovery Partnership (formed by the Substance Misuse Skills Consortium, the Recovery Group UK and DrugScope). The briefing brings together information about older people and substance misuse, including use of alcohol, illicit drugs and prescribed and over-the-counter medications. It uses a range of sources, including: published research and reports; service visits conducted in late 2013; information sent by specialist services, and a roundtable held in October 2013, attended by academics, policy specialists, practitioners, and former service users. The aim is to identify some of the key issues and problems in this area, and to make achievable recommendations for change, so that older people experiencing substance use problems are able to access the support they need. This briefing covers the policy context; routes into substance misuse for older people; risks of and problems associated with substance misuse in older people; barriers to support; positive interventions; and older people and recovery. Case studies give short descriptions of some services which provide effective support for the over 55s. (Edited publisher abstract)
