Personal budgets briefing: learning from the experiences of older people and their carers

This briefing provides a summary of older people's and carers experiences of using self-directed support and personal budgets. It is based on a six month study commissioned from a joint team from Acton Shapiro, the National Centre for Independent Living (NCIL) and the Social Policy Research Unit (SPRU). The briefing covers moving to a personal budget, deciding on personal budget, being assessed, resource allocation, support planning, ways of holding a personal budget, obtaining support, the role of carers, management of the personal budget, the role of external organisations and monitoring arrangements.

This briefing summarises a review of literature and a small-scale survey of good practice on the participation and co-production of older people with high support needs. It looks at the benefits and barriers to participation; highlights two research programmes involving older people with high support needs. Recommendations for improved practice are also listed. It is noted that the evidence base for the review was limited as relevant material was either about participation as it relates generally to older people or was about older people with high support needs but contained very little reference to participation.

The film is introduced by the manager of a residential care home for older people with dementia who talks about the importance of understanding the individual, their interests, background and personal history. The benefits of this approach are illustrated by Marjorie’s story. Marjorie has been supported and encouraged to take up her interest in music again and it has brought pleasure to her life. There is an emphasis on relationship-based, individual person-centred care, rather than a functional approach to basic personal care. This is important to maintaining dignity and well-being. The latter half of the film looks at the family and carer support that the service also provides as part of person-centred care.

This project was commissioned by the Social Care Institute for Excellence (SCIE), to form the basis of practice guidance published by SCIE to support the implementation of proposals in the health and care White Paper, Our Health, Our Care, Our Say. It helped to strengthen the evidence base on how to deliver outcomes-focused services for older people and carers. The project aimed to identify: the service outcomes that were valued by older people and their carers; the organisational and other factors that helped or hindered the delivery of desired outcomes; and examples of good practice in outcomes-focused, person-centred services for older people and their carers. Outcomes' are defined as the impact, or end-results, of services on a person's life; therefore outcomes-focused services are those that aim to achieve the priorities that service users themselves identify as important. The project had two stages. The first covered research on the outcomes valued by older people and their carers; and examples of outcomes-focused practices, including changes in ways of funding, organising or commissioning services, assessment or review arrangements, or the activities of service provider organisations. A postal survey was undertaken to find out how widely outcomes-focused approaches were being developed in services for older people and carers across England and Wales, and the range of different projects or approaches involved. The study then focused on six localities in more depth, examining what changes had been made to the organisation and delivery of services; and the impact of those changes, from the perspectives of service users and carers, managers and front-line staff. The project was supported by an Advisory Group of Service Users, that met at key stages during the project. The Outcomes Network established by SPRU also contributed advice throughout the project.
Keeping personal budgets personal: learning from the experiences of older people, people with mental health problems and their carers

This report is a summary of people’s experiences of using self-directed support and personal budgets. The research was conducted by a joint team from Acton Shapiro, the National Centre for Independent Living (NCIL) and the Social Policy Research Unit (SPRU). The study was based primarily on the experiences of 69 personal budget holders and carers, supported by the views of 40 practitioners and managers from local authorities, and 12 support provider organisations (including five user-led organisations). The findings are structured around the main stages of the personal budget process: moving to a personal budget, including access to information and advice; assessment, self-assessment and resource allocation; support planning and brokerage arrangements; setting up services and managing the personal budget. A final section covers key themes and issues. Whilst examples of positive practice were found, all the case study sites emphasised that it was still ‘early days’ for them in terms of the implementation of personal budgets, and both their personal budgets systems and front-line practice were still evolving.
Supporting black and minority ethnic older people's mental wellbeing: accounts of social care practice

The Social Care Workforce Research Unit, based at King’s College London, was commissioned to undertake a practice enquiry looking at how social care practitioners were supporting BME older people’s mental wellbeing in the four parts of the United Kingdom (UK). The report is based on interviews with over 80 participants who included practitioners (including social workers, commissioners, and workers in the voluntary and community sector), managers, volunteers, BME older people, and family carers. The report sets out some of the background to the enquiry, outlines the methods used and presents findings in seven chapters. Chapters cover: Promoting wellbeing: addressing risk and encouraging social interaction through preventive approaches; mental wellbeing in care homes; personalised approaches to promoting wellbeing and supporting people with depression; housing and housing with care solutions to maintain mental wellbeing; relationships with older people using services, their carers and practitioners; training and skills development; staff views on specialist and integrated support. At the end of each of these chapters, brief summaries and reflections are presented in the form of main messages.

Lifelong couple Brian and Olive live in specialist supported accommodation. Brian has been Olive’s carer since she developed dementia. The housing complex where they live is managed as a community in which each individual is valued for the contribution they can make. The manager explains how important it is for carers as well as the person with dementia to maintain their individual identity and role as partner. The value of having a flexible, responsive individual care plan is also emphasised in the film. Richard, whose mother has dementia, explains how important it was for his mum and dad to be able to stay living together, even when his mum’s needs increased. The film concludes by stating that having well trained and selected staff is vital for achieving personalisation.

This web-based briefing provides a concise summary of the research and policy literature into the means, benefits and difficulties of involving patients in the planning of discharge to community or intermediate care. It also considers the role of carers in this process, as well as what happens when an older person’s ability to communicate their preferences in these matters is affected by dementia, language difficulties, or an unwillingness or reluctance to express preferences about the provision of care. The briefing also examines policy and research findings on older people’s involvement in discharge planning more generally. The briefing was commissioned by the Social Care Institute for Excellence (SCIE).

The material aims to reflect, where possible, the experiences of people with dementia and their family carers. Many of the examples given are located in a care home setting although the issues are also very relevant to supporting a person with dementia in the community.
