Supporting people at the start of their care home journey: a warm welcome

This document provides suggestions on how people who work in a care home can provide a welcome pack for their residents. It aims to help care home staff provide clear information for people covering the issues that matter to them most, and explaining what they can expect from life in a care home. Suggestions for areas to cover include: respecting cultural identity and diversity, communication, social interaction, hobbies and interests; involving family and the local community; practical arrangements for day-to-day living; health care; costs; and making a complaint. This report provides information about the Care and Social Services Inspectorate Wales (CSSIW) findings from looking at adult care homes in Wales. Care homes provide care together with accommodation for both younger and older adults. Homes varied in size and provision; they could care for people with learning or physical disability, mental health needs, sensory impairments and older people. Overall there was evidence of continued improvement in several core areas: the quality of care planning has improved; service users' wishes and feelings are taken into account and there is evidence of service users being involved in decision making regarding their care; service users' privacy and dignity is promoted in the home; the maintenance and cleanliness of premises has shown some improvement; recruitment practices including pre-employment checks on staff have improved; providers taking responsibility for reviewing the quality of care provided; the appropriate assessment and management of risk to service users. The following key areas still require attention: support and training for staff; staffing levels; supervision of staff; and appropriate management of medication must still be a priority for care homes. There was a decrease in both the number of adult protection issues referred to CSSIW and CSSIW's subsequent involvement in investigations. This may in part link to the developing understanding of the role of CSSIW in adult protection.

The OFT report reflects that there are a number of different players with roles in ensuring the provision of good quality care home services for older people. This is helpful in emphasising the shared responsibility for all the players in resolving the issues identified. The Government is keen to establish what we can do, working with others, to address these issues most effectively. This response is therefore the start of an ongoing dialogue that we hope to have with delivery partners.. It is the job of central Government to set the strategic direction for social services and produce national guidance. However, a number of the issues identified in the OFT report relate to the inconsistent application of current legislation and DH guidance. This raises the issue of inspection and improvement.
