Motiveless malignity: problems in the psychotherapy of psychopathic patients

This article attempts to draw attention to the difference between the states of mind and inner worlds of neurotic, borderline and psychopathic patients, with reference to different types of destructiveness: anger in the neurotic patient; desperate vengeful hatred in the borderline paranoid; and a cold addiction to violence in the psychopath. Although most patients refuse to stay put in the neat schematic categories outlined, they do seem to appreciate and to need the therapist's recognition of the specific quality of these vastly different states of mind.

Over recent years, abuse of vulnerable people such as elderly with dementia, those with a learning disability or mental health problem has received increased publicity. This article outlines guidelines prepared by the charity United Response to guide and assist all staff, who may need to know how to act when abuse is alleged and also to assist staff in identifying signs that abuse may be occurring or that a situation exists which might lead to abuse. Covers the need for staff training, definitions of abuse and necessary procedures.

Organised violence and the stress of exile - predictors of mental health in a community cohort of Vietnamese refugees

Using a model which included variables from different research traditions the prevalence and course of mental disorders among Vietnamese refugees were studied. From the research it was concluded that the effects of war and persecution were long-lasting, and compounded by adversity factors in exile. Observes that a uniform course of improvement in mental health after resettlement cannot be expected in all contexts and recommends that the affected refugees need systematic rehabilitation.

In some occupations, there is an increased risk of becoming a victim of a violent incident. Nursing and care work, in settings for people with learning disabilities and challenging behaviour, or who have mental health needs, are occupations which can involve such a risk. This study examined how staff felt, after a violent incident, towards the perpetrator of the incident, and the reasons that the care staff gave as to their belief about the cause of the incident. Coping was investigated by looking at measures of anxiety.

Concepts such as Post Traumatic Stress Disorder serve to distract us from the simple truth that many people suffer lives which are almost continually traumatic. In the case of the person described in this paper it might be asked why she did not react in a violent way before the incident which led to her incarceration in a secure hospital. Argues that both psychiatry and psychology serve to further detract from a realistic understanding of difficulties faced by many clients living traumatic lives.
