Blowing the whistle on bad design

This publication provides unit costs for a wide range of health and social care services using a standardised methodology. The report is organised into five main sections. Section I covers services used by a particular client group, and includes services for older people, people with mental health problems, people who misuse drugs/alcohol, people with learning disabilities, younger adults with physical and sensory impairments, services for children and their families, hospitals, and care packages. Sections II, III and IV deal with the unit costs of professionals in community-based health care, community-based social care, and hospital-based health care. The population characteristics of people with severe and enduring mental health problems living in 'high support' Health and Local Authority (LA) residential facilities were compared in this survey of social functioning and psychiatric symptomatology. The survey was intended to assist in local service planning, particularly in the targeting of relatively high cost provision. A cross-sectional survey of all 62 residents of the three Local Authority hostels was conducted and compared to an earlier survey of 76 residents living in seven NHS residential facilities. Residents of NHS facilities were found to have poorer social functioning than LA residents; had more physical health problems; had higher levels of behaviour problems and risk behaviours which were both rated as more serious in terms of their effect on community tenure. The survey's findings were consistent with the results of other surveys of the characteristics of people considered 'hard to place' in the community as a result of mental health problems.

The remit of the working party was to explore options for addressing the issues that currently prevent older people in care homes receiving the appropriate level and type of health and care services. It concludes that an integrated interdisciplinary approach is required. Key issues include the need for: a standardised interdisciplinary approach to assessment, care planning, and care delivery; development of the nurse as the lead practitioner in care homes; comprehensive systems of service delivery to engage general and specialists; and for all practitioners engaged in care home practice to have appropriate education and training and for relevant programmes of research.
