Understanding the mental health problems of children who have learning disabilities presents huge challenges across many domains. The paper examines some of the assessment frameworks that may be used, offers clinical guidance on collecting information and suggests ways in which various theoretical approaches can be used to derive comprehensive case formulations. The common assessment approaches mentioned are psychiatric, behavioural, ecological and early attachment. A case study is used to discuss the use of the frameworks. An emphasis is placed on the use of structured methods to improve reliability and validity.
Prematurity at birth and adolescent depressive disorder

Association between prematurity/low birthweight and adolescent depressive disorder studied using a case–control design within a prospective cohort study of 2032 adolescents. Odds for depressive disorder were 11-fold (95% CI 2–62) higher for the premature/low-birthweight participants after regression adjustment for major confounding factors. For premature/low-birthweight females, cumulative rates of depressive disorder over 30 months were 15.2% (95% CI 11.1–20.5) v. 1.8% (95% CI 1.6–2.1) in those with normal deliveries. Physiological adaptations in utero before full term may be implicated causally in some cases of depression in adolescence.
Psychosis and academic performance

It has been suggested that psychosis genes might be associated with beneficial effects, explaining their high frequency in all human populations. The unusually complete demographic and scholastic records available in Iceland were used to locate academically accomplished individuals and assess the probability of previously identified patients with mental disorders and their relatives being among such groups. Close relatives of successful students showed increased risks of psychosis. Individuals who subsequently developed psychosis and relatives of people with psychosis excelled in school performance, particularly in mathematics. The study supports the hypothesis that stimulation associated with psychotic tendencies enhances performance in academic settings.
Prescribing practices of community child and adolescent psychiatrists

A questionnaire was sent to all 55 community child and adolescent psychiatrists working in the West Midlands to examine the prescribing practices of community child psychiatrists, because most previous studies have included prescribing data from specialist (tier 4) services. The survey had a good response rate (87%). The vast majority of child psychiatrists were prescribing stimulants and antidepressants, and over half were prescribing antipsychotics and melatonin. At least half of the consultants would consider prescribing an antipsychotic for the treatment of aggressive behaviour. One-third of consultants cited pressure on services as a reason for prescribing medication. Community child psychiatrists were prescribing more medication than expected, which may have a considerable impact on referral rates to adult services in the future. It is concerning that the increased use of medication may be a reflection of pressure on services.

Electroshock or electro-convulsive therapy (EST/ECT) is an invasive psychiatric procedure aimed at alleviating symptoms rather than achieving cure. ECT recipients and some practitioners are actively promoting the ban of the procedure. Offers a brief summary of how ECT originated and presents self reports from teenagers given electroshock, sometimes non voluntarily.
