Trencin statement on prisons and mental health: adopted in Trencin, Slovakia on 18 October 2007

The Trencin Statement draws the attention of all countries in Europe to the essential need for greater focus on mental health problems among people in custodial settings. Of the nine million prisoners world-wide, at least one million suffer from a significant mental disorder, and even more suffer from common mental health problems such as depression and anxiety. There is often co-morbidity with conditions such as personality disorder, alcoholism and drug dependence. The Statement is a product of the WHO Health in Prisons Project work during 2007 where the focus was on mental health in prisons across the European Region. The project organized a mental health conference in Trencín, Slovakia in October 2007 where the Statement was adopted by the 28 European Member States that were present at the conference.

Substance use is common in a wide range of psychiatric disorders, which can increase the risks of use developing into a substance use disorder. Conversely, substance use disorders are frequently accompanied by psychiatric symptoms. In some people these indicate formal psychiatric disorder. Whatever the causes, comorbidity may complicate treatment and resolution of both disorders, often presenting as a vicious circle that is difficult to break.

Reviews the literature on the associations among childhood violence, perpetration of partner violence, presence of alcohol problems, and presence of psychiatric problems in males; and synthesizes this literature into a theoretical model of the associations among childhood violence, alcohol problems, and perpetration of partner violence for men. History of parental alcohol and psychiatric problems, antisocial behaviours during childhood and adulthood, depressive symptomatology, and marital conflict are hypothesized as additional causative and mediator variables for alcohol problems in this model. Antisocial personality disorder is also considered as a moderator variable for the association between partner violence and alcohol problems, but is criticised on both empirical and theoretical grounds.

This article describes a study that seeks to determine the extent to which nine psychometrically sound measures of problems with social functioning account for seven different reliable and valid measures of problems with social functioning. The strong psychometric characteristics of the measures used make it possible to examine comprehensive predictive models, and this provides insight into the ways that the social environment affects personal adjustment.
