Development and preliminary testing of a framework for quantifying local service provision for people with dementia

Purpose: It was hypothesised that there were variations in health and social care services available for older people with dementia and their carers, and that measurement of this between localities was possible. The purpose of this paper is to present a framework for examining this. Design/methodology/approach: Using a case study approach, data from national surveys of local authorities providing social care and National Health Services Trusts providing old age mental health services conducted in 2014/2015 in England were used. From these, indicators of variation in services for people with dementia and their carers in different geographical areas were created. Measurement of the presence/absence of each service permitted the creation of a service mix score for each area. Findings: The framework comprised 16 attributes each with indicators describing the characteristics of the organisations providing the services; the skill mix of community mental health teams for older people; and the health care and social care services available in localities. Variation was evident, confirmed by quartile analysis and exemplars, suggesting that older people with dementia and their carers in different localities are likely to experience differences in the range of provision available, particularly social care services. Originality/value: The case study approach used achieved its objectives, and the resultant framework has potential for generalisability and utility, given acceptable ecological validity and discriminant validity in identifying variations in service mix. The UK Inquiry into Mental Health and Well-Being in Later Life has published its final report, Improving Services and Support for Older People with Mental Health Problems. The report follows the Inquiry's first report, Promoting Mental Health and Well-Being in Later Life, which was published in June 2006. The Inquiry has worked in two stages: Stage 1 focused on the promotion of good mental health and well-being in later life. Stage 2 examined services and support for older people with mental health problems.

The Department of Health has commissioned this review of ageism and age discrimination in the provision of mental health services for older people in the context of the European Commission Draft Directive (July 2008) - COM (2008) 426 and the passage through the United Kingdom parliament during 2009‐10 of the Equality Bill and related secondary legislation that will outlaw age discrimination in the provision of goods and services, including health and social care. Topics include: ageism and types of age discrimination; stigma, stereotypes and ageist attitudes; mental health services for older people; age discrimination in the treatment of particular conditions: common mental disorders, dementia, less common conditions, comorbidity; prevention and public health interventions; NICE guidelines and QALY; education and training. This review is one of four reviews of ageism and age discrimination in health and social care.

This article analyses the key elements of 'Everybody's Business', published by the Care Services Improvement Partnership (CSIP), so as to provide a checklist of useful issues and actions to take into consideration when developing and delivering services for older people with mental health needs. Other key publications are also identified. The guidance is felt to be comprehensive expect for one key area: the aspirations of the service users and placing them in the context of family, friends and the community. The need to take into account the whole person, not just whole systems, is identified and a way of building this into the Department of Health's model is suggested.

The services currently offered are patchy, so there is a pressing need for effective mechanisms to be introduced in order to monitor quality.
