The article contains an interview with Shaun Gravestock, a full-time consultant psychiatrist at the newly opened Mental Health and Learning Disabilities at the Bethlem Royal Hospital in Beckenham, Kent. This unit caters specifically for people with learning disabilities who need intensive mental health care. It contains 13 beds, 9 funded by local primary care trusts and 4 available to commissioners around the UK. Gravestock argues that mainstream acute mental health units are not the best places for potentially vulnerable people with learning disabilities, as staff may find it difficult to establish rapport, the atmosphere can be volatile, and staff are under pressure to quickly move patients through the system. The Bethlem unit aims to fill this gap for a specialist service for those with learning disabilities and mental health problems.
Service user views of long-term individual psychodynamic psychotherapy

Long-term psychodynamic psychotherapy is a costly service to provide, but many clinicians believe it is of benefit for people who have learning disabilities and psychological problems. There is also now some evidence for its effectiveness. However, the views of recipients is unknown. In this study, recipients of more than two years of psychodynamic psychotherapy were interviewed about their experiences and views. Themes emerged about the referral process, the experience and the outcome. Areas of strength were identified, as well as areas for improvement. The findings concur with previous findings on group therapy and help inform current and future provision of long-term psychodynamic psychotherapy.

Many parts of the world are developing specialist mental health services for people with a learning disability. Government policy in England appears to favour a move in the opposite direction. The general aims of mental health services for people with a learning disability are indeed similar to those of the rest of the population, but distinctive clinical skills are required to assess, treat and support effectively people with a learning disability who have mental health and/or behavioural problems. It is argued that there is therefore a need for specialist services to meet the needs of this population, which should include acute admission facilities, outreach services in the community and long-term support.

The relationship between challenging behaviour and mental ill-health in people with intellectual disabilities: a review of current theories and evidence

Challenging behaviours and problems of mental ill-health are common amongst people with intellectual disabilities. The article examines conceptual similarities and differences between these conditions, examines the data on comorbidity, and explores possible hypothetical relationships between behavioural and psychiatric disorder in this population. While there is little evidence at present to suggest that many of the challenging behaviours seen in people with intellectual disabilities are underpinned by problems of mental ill-health, only qualified conclusions are possible because of limitations in the quality and scope of existing research. A conceptual model for looking at the risk variables that may contribute to both conditions is suggested, and requirements for future research and current multi-disciplinary practice are outlined.

This article explores the spiritual dimension in mental health care and its relevance to people with learning disabilities. A discussion of the development of a working group to scope the issues in one learning disability service is explored, with practical commentary on the efforts made to gather more information from service providers. The findings from a brief survey are briefly discusses, as well as developments which have ensued to date. The authors conclude that, although spirituality can mean different things to different people, responding to the diverse spiritual needs of service users must take a person-centred and a flexible approach.
Arson: characteristics and predisposing factors in offenders with intellectual disabilities

This retrospective study focuses on examining the characteristics of offenders with intellectual disabilities (ID) and the range of identified reasons for the offence. The study population consisted of 1,100 patients with ID who were in contact with psychiatric services at the Leicestershire Frith Hospital. Fifteen patients were identified as having committed arson. The findings indicate a higher prevalence of arson in this population, along with the fact that the majority of people with ID who have committed arson tend to bypass the criminal justice system. A significant number are likely to repeat the behaviour and will also commit other offences. The most common reason for arson appears to be revenge, closely followed by suggestibility. The majority has an associated diagnosis of personality disorders along with Axis 1 psychiatric diagnosis. Other factors include large family size, history of childhood psychiatric disorders, abuse, homelessness, unemployment and relationship difficulties.

The mainstreaming of students with mental health and/or learning support needs means further education college tutors are carrying an increasingly heavy burden of social and emotional care, often in crisis situations. This argues that colleges should do more to support their tutors in their pastoral role.
