Quality in social care: achieving excellence in supported living services

In this film we see several examples of excellence in supported living settings. The film shows people with learning disabilities and mental health problems finding employment, developing skills, and becoming more independent, and looks at the role of social care staff in supporting people to improve their lives. The examples are analysed by a panel of service users and social care academics and they discuss how the services demonstrate concepts of excellence in social care in their day-to-day work. This film was previously available under the title 'Defining excellence: residential settings: supported living' which was revised in 2014.

An annual statistics report on the use of the Adults with Incapacity (Scotland) Act 2000. The report examines the geographic variations in the use of welfare guardianship, the age and diagnosis of people placed on guardianship, the duration of guardianship orders applied for by applicant, variations in indefinite orders by age and diagnosis, and geographic variations in orders approved on an indefinite basis. It shows that the number of new and existing orders continued to rise although there was a further significant reduction in the granting of orders on an indefinite basis. The percentage of orders granted where the cause of the adult’s incapacity was dementia fell to 45 per cent, down from 46 per cent the previous year. Conversely, there was an increase from 41 per cent to 44 per cent of orders where the incapacity was caused by a learning disability, reflecting a continuing trend over the past several years where welfare guardianship is being used for an increasingly younger population. Adapting to trauma: disengagement as a holding strategy

Purpose: This paper draws upon a range of material to improve the understanding of disengagement with everyday life, by some individuals who have learning disabilities and mental health difficulties. Illustrative incidents from historical clinical cases are utilised, to consider whether this reframing may enhance the interpretation of presenting behaviours. Design/methodology/approach: Key recurring themes within transpersonal literature were reviewed, relevant to adults with behaviour indicating a degree of disengagement from everyday life. These were grouped into Physical Realm, Psychosocial Realm and Realm of Being. Illustrative examples of behaviour are reviewed and re-interpreted within this framework. Findings: These examples generated plausible interpretations for the presenting behaviours within this framework of the Three Realms. These interpretations support a fresh understanding of the quality of the individual's inner experience. This paper suggests a potential framework to consider the way in which some individuals may experience a different quality of consciousness than the usual. Practical implications: Use of the Three Realms for behaviour interpretation should result into a more empathetic and client-centred approach that could reduce the need for aversive approaches, lessening risk for the client and any employing organisation. The identification of behaviours that signal participation in the Realm of Being could be defined and evaluated with the potential to be used to inform the nature and content of the support provided. A review of risk factors associated with suicide in adults with intellectual disability

Purpose: As recently as the 1980's it was still a widely held belief that individuals with intellectual disability (ID) did not have the cognitive capacity to experience mental health problems and this acted as a “buffer” against suicidal behaviour. The literature review examines the evidence relating to risk factors for individuals with intellectual disabilities. Design/methodology/approach: The literature review covered the time period 1993-2013 and returned 117 studies, 77 of which related to individuals with ID, 37 of which related to adults. Following screening titles and abstracts 28 articles were removed. A total of nine studies were found to be eligible for inclusion in the review. A further two studies examining suicide in adolescents (up to adulthood) with ID were also considered. From the eligible studies the following information was considered: study design, sample size, strengths, limitations and the risk factors associated with an increased risk of suicide. Findings: The suicide risk factors identified during the review were found to be in keeping with the general population and included a diagnosis of clinical depression, history of self-harm, unemployment, loneliness, unemployment, an increased need for support from others, early onset mental illness and being treatment resistive. Youthful offending and delinquency: the comorbid impact of maltreatment, mental health problems, and learning disabilities

This paper reviews the links from mental health disorders, learning (and academic-related) disabilities, and maltreatment victimizations to delinquent and serious youthful offending behaviours, and provides successful collaboration outcome examples to address this problem. The most difficult challenge to juvenile courts and youth-caring systems is working with adolescents and families with comorbid difficulties, for this group is most at risk for incarceration and involvement with the adult criminal justice system. Liaison and diversion services aim to identify and support people with mental health problems, learning difficulties and other vulnerable people in police stations and courts. This report identifies the key elements of successful liaison and diversion services in six services based in Lewisham, Manchester, Portsmouth and in Plymouth, Bodmin and Truro. The findings are based on visits to sites, interviews with staff, service users, and partners. It finds that the most successful teams offer support for a wide range of a person’s needs, they build packages of support from a range of local agencies, and they stay in touch with people after they have been referred to other services. The report then makes recommendations for NHS commissioners and liaison and diversion services. (Edited publisher abstract)
The Transinstitutionalisation of People Living in Licensed Boarding houses in Sydney

Deinstitutionalisation, one of the most significant social policies of the last century, was introduced into Australia from the 1960s, and involved the closure or downsizing of large institutions and the integration of people with disability and mental illness into the community. One of the community-based accommodation options for people leaving institutions was boarding houses. This paper presents the findings of a study into the use of licensed boarding houses for people with intellectual disability and people with mental illness in Sydney, Australia. The study aimed to explore how boarding houses operate as an accommodation option within the policy of deinstitutionalisation and the extent to which this accommodation type upholds human rights principles and contributes to residents' quality of life. Forty interviews were conducted with a range of participants including current and former residents, proprietors of licensed boarding houses, and staff of community organisations and government agencies about licensed boarding houses and the quality of life of residents. The study found that licensed boarding houses are a form of transinstitutionalisation and do not serve to enact the human rights principles articulated in current policy and legislation. The importance of being earnest: our experience of involving service users with complex needs in staff recruitment

This article describes our experience of involving people with learning disabilities and additional needs, such as behaviour that can be challenging or mental health conditions, in the recruitment of staff. A significant amount of support, as well as additional time and adaptations to the interviewing and scrutinising processes were necessary in order to ensure that service users could contribute meaningfully. However, the process was not only really valuable for users and staff alike but also, in our view, efficient in choosing a robust applicant. (Edited publisher abstract)
Assessing risks arising from challenging behaviours: a model and a brief review of existing resources

Purpose: Challenging behaviours are defined in part by the risks of harm they present and the support of individuals who challenge would be improved with effective assessments of the risks posed by challenging behaviours. The purpose of this paper is to outline a model of risk assessment and review literature that provides relevant information. Design/methodology/approach: Literature review. Findings: Forensic risk assessment has a well worked-out and validated approach but tools used to assess challenging behaviours are not effective risk predictors. Applied Behaviour Assessment provides methods that can be more effectively used for risk assessment. Practical implications: There is a need to develop valid risk assessment procedures for assessing the risks presented by challenging behaviours. Originality/value: This is a fresh perspective on risk assessment in relation to challenging behaviours. (Publisher abstract)
