The Department of Health's publication of the "Breaking Down the Barriers" strategy for development sets out to identify practical ways in which existing counseling provision can be improved and increased. This report is an evaluation of the 'Working Together in London project', set up by the Sainsbury Centre for Mental Health, the King's Fund and the DoH to support people with severe and long term mental health problems. This project examined the work of three assertive outreach teams in the capital. Supporting someone with a mental health illness and substance misuse problems - alcohol and/or drugs - is one of the biggest challenges facing frontline mental health services. The complexity of issues makes diagnosis, care and treatment more difficult, with service users being at higher risk of relapse, readmission to hospital and suicide. One of the main difficulties is that there are a number of agencies involved in a person's care - mental health services and specialist rehabilitation services, organisations in the statutory and voluntary sector. As a result care can be fragmented and people can fall down the cracks. The guidance provides a framework within which staff can strengthen services so that they have the skills and organisation to tackle this demanding area of work. The authors key message is that substance misuse is already part of mainstream mental health services and this is the right place for skills and services to be. Mental health services must also work closely with specialist substance misuse services to ensure that care is well co-ordinated. The authors have also highlighted some existing examples of excellent services working well now. This guidance and the services mentioned, demonstrate the importance of effective leadership at a local level and rigorous training programmes to help staff maintain high standards of service delivery. The Health of the Nation Outcome Scales for People with Learning Disabilities (HoNOS-LD) is the result of collaboration between the Royal College of Psychiatrists, the British Psychological Society and the Department of Health. HoNOS-LD measures global outcomes (improvement or deterioration) in people with learningdisabilities who have additional mental health needs.

Across London, the development of local mental health and well being strategies has been inconsistent. Co-ordinating partnerships and engaging agencies in the process is a challenge, and often there has been little clarity about what mental health promotion is, and what it can achieve. Low levels of commitment, small dedicated resources and poor profiles make sustaining the work difficult. Organisational change poses additional challenges, as it can threaten continuity. Three key areas demand immediate attentio are:· greater prioritisation of the promotion of mental health and well being, co-ordination of policies and practices; and integrated, effective commissioning of programmes and projects promoting mental health and well being.

Although the impact of increased income on individual health measured in this study is modest, the benefits of such small improvements to deprived people with chronic conditions should not be underestimated. This study lends support to the provision of welfare benefits advice in primary care as part of a holistic approach to the care of such patients.

The goal of this training programme was the development of child welfare practitioners and community professionals who have the ability to respond effectively to complex family problems of child abuse and neglect resulting from substance abuse, mental illness, and domestic violence. A quasi-experimental design utilizing pre- and post measures was used to test the impact of interdisciplinary training. The evaluation shows gains in trainee knowledge, positive attitudes toward collaboration, and the trainees reported more collaboration in their own practice between the pre- and post tests that was sustained in a follow-up.

This American article describes an 8-week training curriculum conducted jointly with parent employees and professionals in an agency providing services for children with serious emotional disabilities. The collaborative seminar was part of one rural programmes implementation of a family strengths based system of care in children's mental health. Describes the curriculum and provides an overview of lessons learned and implications for future parent–professional joint training.
