Childhood abuse and cannabis use among adolescents with mental health needs in Ontario, Canada

Objective: The purpose of this study was to examine the association between childhood abuse (emotional, physical and sexual abuse) and cannabis use among adolescents with mental health needs. Methods: Data on 3681 adolescent in-patients, 12–18 years old, were obtained from the Resident Assessment Instrument for Mental Health (RAI-MH). Using logistic regression, we estimated the odds of using cannabis by adolescents who experienced childhood abuse after controlling for age, gender, Aboriginal origin, problems with addiction, history of criminal justice involvement and symptoms of depression and mania. Results: There were 1844 adolescents, representing 50.1%, who reported using cannabis within the last 12 months. Controlling for demographic and patient characteristics, we found that cannabis use in the past year was strongly associated with childhood sexual and physical abuse. Compared to non-abused females, females who experienced sexual and physical abuse were more likely to have used cannabis. For males, the experience of physical abuse was marginally associated with cannabis use. Conclusion: The current data demonstrate the strong association between childhood sexual and physical abuse and cannabis use with a particularly strong association for females. Efforts aimed at treating cannabis use in adolescents who present with mental health needs should also consider their abuse histories. (Publisher abstract)
Adoption preparation differences between adoptive families of children with and without special needs

A sizable proportion of special needs adoptions result in a disruption/dissolution or a displacement. Researchers emphasize the importance of comprehensive preparation to support adoption adjustment. This article examines the differential receipt of preparatory services based upon child and family characteristics and the child's risk history. Analyses of three types of preparatory services identified through factor analysis, including general, biological, and behavioral information/ services, confirm differential preparation of adoptive families. However, three variables were predictive of receipt of all types of preparation, including type of adoption, kinship status, and pre-adoptive risk history. Summarises the evidence on the links between mental health and drug use, and reports some welcome recent advances in treatment. Argues that the double exposure of people with dual diagnosis to social exclusion demands greater attention to the questions of cause and effect if this rapidly escalating problem is to be checked, not simply pathologised and contained.

Health status of juvenile offenders: a survey of young offenders appearing before the juvenile courts

Reports on the health needs of a sample of juvenile offenders appearing before Manchester Court during the month of August 1992. Of the subjects interviewed, 19 percent had significant medical problems, 42 percent a history of substance abuse and 7 percent psychiatric problems requiring further treatment. One quarter engaged in a variety of dangerous behaviours. Custodial remands were common, particularly in those with multiple psychosocial difficulties.

People with a dual diagnosis of mental health and drug or alcohol abuse problems may fall between existing specialist services, which tend to focus on treating one or other problem. Argues the case for a flexible, individualised approach to treatment which acknowledges the biological and psychosocial features of the co-existent problems and draws on clinical knowledge and skills from both the mental health and substance addiction professions.
