The development and evaluation of a hybrid decision support system for clinical decision making: the case of discharge from the military

This article reports on the development, implementation, and evaluation of a decision support system (DSS), a hybrid of a statistical model (regression equation) and expert rules, to support this decision. The study focused on the decisions made by social workers, psychologists, and psychiatrists serving as military mental health officers to recommend discharge from compulsory duty in the Israeli army because of mental or emotional difficulties. The validity of this hybrid DSS was established. User acceptance was low. This article discusses these findings and their implications for practice.

Compulsive hoarding behaviour, exhibited in late life, can have serious implications. Geriatric care managers and practitioners in Older Adult Protective Services often grapple with issues related to ethics and ageing, including client self-determination, that are raised by hoarding actions. Involuntary intervention may be needed to insure that the older person is not in imminent danger. Case examples are used in the article to illustrate the behaviour as well as provide outcomes.

This article considers the relevance of paternalism in the care of people with impaired capacity. The author uses the finding of the Ritchie report as a vehicle to ask whether promoting autonomy may require the measured use of paternalistic intervention.
