Although community mental health teams (CMHTs) have become the main vehicle for coordinating health and social care for people with severe and long-term mental health problems, little research exists on their management. This paper reviews the available literature with reference to the wider literature on innovation and organisational behaviour and examines strategies for improving CMHT management at macro and team level. The challenges of researching the topic are described with recommendations for further research based on social constructivist theorising and triangulated methodology.
Predictors of depressive mood, occupational stress, and propensity to leave in older and younger mental health case managers

Age differences and relationships between occupational stressors, work environment, depressed mood, and propensity to leave were examined among 263 case managers (CMs) in community-based mental health agencies. For younger CMs, satisfaction with the supervisor, perceived job pressure and lack of organizational support were significant predictors of propensity to leave the job. Older CMs who reported lower satisfaction with financial rewards, the type of work they did, and greater use of avoidance/resignation and help seeking coping strategies were more likely to rate themselves higher on the depressed mood scale.

The past decade has seen an increasing focus in the community. During this period, the media has given a high profile to a number of incidents where members of the public and health service staff have been assaulted by patients of mental health services. Presents a systematic review of 10 studies, two conducted in the United Kingdom and eight in the United States of America. It is suggested that the occupational therapy profession urgently needs to address this gap in its literature and consider what can be learned from other professions.

Intensive case management for people with serious mental illness - Site 2: cost-effectiveness

Evaluates the cost-effectiveness of intensive case management (ICM), compared with standard psychiatric services. Results found that in comparison with standard psychiatric services, ICM clients received more psychiatric out-patient, primary health, residential and social services care, in addition to high levels of input from case managers. The ICM group therefore cost nearly three times as much as the control group. ICM teams may be necessary to keep contact with some clients, but achieve very limited clinical or social outcome, at high cost. Concludes that the cost could be reduced by either adjusting team skill mix, varying intensity of contact with clients or by a corresponding reduction in the use of in-patient care.
