Narrative inquiry on case studies of crisis in dementia

Purpose: This study aims to explore the coping styles that can be inferred from the discourse of dyads with dementia, and how these appear to impact on care management. Design/methodology/approach: This was a case study approach. Participants were recruited from two teams managing crisis in dementia in the UK. The authors conducted multiple qualitative interviews with people with dementia and their family carers over the course of one month. The analysis was first performed through thematic analysis. Data were further analysed through narrative inquiry to create a story line, or play in our case, for our findings. Findings: Five dyads were interviewed and a total of 16 interviews were conducted. Three dyads were husband–wife and two were daughter–mother relationships. The mean age was 67.4 years for carers and 79.8 years for people with dementia. In these cases, the carer assumed responsibility for managing the episode and was more likely to seek formal help if a pre-existing plan was in place. Otherwise, when a crisis arose, dyads preferred to avoid involving professionals. Practical implications: Psychosocial interventions should aim to identify and replace unhelpful strategies used by dyads to manage crisis episodes. Originality/value: To the best of the authors’ knowledge, this is the first study using qualitative interviews of dyads to inquire into their experience of mental health crisis. This report aims to help local areas plan and deliver specialist services, led by old age psychiatrists, to meet the needs of older people in the context of the NHS Long-Term Plan. It examines the demographic and epidemiological imperatives within health and social care and sets out the case to focus on addressing the mental health needs of older people within integrated systems. It includes the views of older people, service users, carers and a wide range of health and social care professionals. It also highlights good practice and innovative service examples involving old age psychiatrists across different health and social care settings, including in care homes. It also sets out next steps for those developing and implementing policy, strategy and service changes within the context of the Long-Term Plan in England. (Edited publisher abstract)
Ethnic minority, young onset, rare dementia type, depression: a case study of a Muslim male accessing UK dementia health and social care services

A case study comprised of formal interviews, formal observations and informal discussions investigated the motivations and experiences accessing dementia care health and social care services for a Muslim, Pakistani male with dementia. Motivations derived from ‘desperation’ and an inability to access support from family or religious community. Experiences of accessing services were mostly negative. Dementia services were ill-informed about how to support persons with young onset dementia, with pre-existing mental health conditions, from an ethnic minority. Education and training to remove barriers to all dementia care services is required for persons with dementia, their families and within dementia services and religious communities. Leaflet from a group of housing associations which highlights different approaches to tackling mental health issues in the West Midland. It provides brief descriptions of Midland Heart's Mental Health Step Down Service for people discharged from an acute mental health setting, Walsall Housing Group's Young persons housing project, which supports vulnerable young people maintain their independence; Nehemiah UCHA Retirement Living Service which supports black and minority ethnic older people, including those with dementia, to live independently; and the Fry Housing Trust's Support and resettlement service, which provides supported housing to vulnerable homeless adults who have multiple and complex needs. The examples emphasise the positive value of health and housing partnerships, and the savings and successful outcomes they bring for individuals. (Edited publisher abstract)

Each section includes five top tips from a leading organisation in the sector and a practice example to show how these tips can be put into in practice to improve the quality of life of people living with dementia. Organisations contributing to the guide include Vista Blind, Rethink Mental Illness, Making Space, Hull City Council and the Fremantle Trust. This article is about NHS Health Scotland's Mental Health and Wellbeing in Later life Programme, which aims to promote mentally healthy ageing in Scotland. A core aspect of the programme is involving older people in the design, delivery and evaluation of health improvement projects. The article presents three case studies: the use of peer researchers to facilitate focus groups in Glasgow; focused action research in Lothian care homes; and the production of Facing Dementia, a booklet based on real experiences and featuring quotes from people with dementia. The article concludes by outlining future plans, including developing an action plan focused on longer-term priorities for promoting mental well-being.
