Improving the detection and assessment of mental health problems in people with learning disability

Ensuring that people with learning disability get appropriate help for their mental health problems is not just a job for psychiatrists. All staff have an essential role in ensuring that people who have psychiatric problems are identified and referred for comprehensive assessment. This article introduces some of the major issues in recognising and assessing mental health problems in this population, and introduces some new developments designed to improve the targeting of psychiatric referrals.

The risk taking approach has been developed by practitioners who wish to involve and empower mental health service users. In this article the author argues that risk taking is an essential element of working with mental health service users to ensure autonomy, choice and social participation.

Anxiety disorders in dementia sufferers

One hundred and fifty-eight consecutive patients attending a university memory clinic were assessed using variety of standardized instruments. Dementia was diagnosed according to DSM-III-R criteria. One hundred and nine patients had DSM-II-R dementia. None of the cognitive or demographic variables were significantly associated with the presence of anxiety symptoms. Three main categories of anxiety symptoms were evident - anxiety related to depression, anxiety related to psychosis and anxiety to interpersonal situations.

Examines the construct validity of the Trauma Symptom Checklist-40 (TSC-40) in a sample of female psychiatric inpatients. Consistent with other findings, the TSC-40 displayed criterion-related validity in relation to childhood sexual abuse. Survivors of sexual abuse obtained significantly higher scores than those without such a history except in the Depression subscale.

Clinical interviews provide occupational therapists with sensitive and versatile sources of information; however, reliable means to collect and interpret this information are necessary for effective clinical decision making. The Occupational Performance History Interview (OPHI) is a semi-structured interview, designed to address the need for a reliable and clinically useful tool in occupational therapy. This article describes a qualitative research study, in which four experienced therapists reflected on their use of the OPHI to interview people referred to a psychiatric day hospital service.
Reliability and validity of a measure of sexual and physical abuse histories among women with serious mental illness

Describes a standardised questionnaire of histories of abuse among women with serious and persistent medical illness, and assesses its test-retest reliability and its validity. Results showed that childhood histories of physical and sexual abuse can be reliably and validly assessed in women with severe and persistent mental illness using a standardised instrument.
