Improving equality of access to Independent Mental Health Advocacy (IMHA): a report for providers

This report aims to help Independent Mental Health Advocate (IMHA) services reach everyone who is entitled to their support. It aims to help IMHA providers to achieve the best possible outcomes for all people treated under the Mental Health Act 1983 amended in 2007. Drawing directly on the findings and recommendations of the national review of the quality of IMHA services in England, the report highlights the problem of unequal uptake of IMHA by some groups of ‘qualifying patients' and explores what can be done to change this situation. It highlights the obligations of IMHA services under the Equality Act (2010) and provides concrete suggestions about how to take effective action and improve practice when working with people sharing relevant protected characteristics. It also suggest steps that IMHA providers can take to help them identify, understand, and address the barriers to the full and effective use of their service by everyone who is entitled to access it. This summary aims to help Independent Mental Health Advocate (IMHA) providers to open up their service to everyone who has the right to use it. Ensuring equalities within IMHA services means reaching all qualifying patients regardless of their ethnicity, age, gender, disability, beliefs, sexual orientation or any other characteristics protected by the 2010 Equalities Act. A fifth of women experience mental health issues in the perinatal period, yet services to address this remain patchy. This article discusses how implementing such services for pregnant women and new mothers could save money and lead to better outcomes for those affected. It looks at the costs of poor perinatal provision to society as a whole, the need for better training for doctors and midwives, and the option of setting up a perinatal psychiatric team within maternity services. The majority of patients detained under the Mental Health Act 1983 are eligible, under section 130 of the 2007 Act, to access Independent Mental Health Advocate (IMHA) services. However research has shown that less than half of those qualifying for an IMHA appear to be accessing them. This summary and flowchart provide the essential information needed to implement an open access policy. Open access means qualifying patients are automatically referred to IMHA services unless they object. This approach has implications for IMHA service capacity; resourcing; consent and confidentiality. Access to services by children with intellectual disability and mental health problems: population-based evidence from the UK

Background: Children with intellectual disability (ID) experience high levels of mental health problems that may require access to professional support. However, there is limited information about how frequently these children access services. Method: A survey of mental health in children living in the United Kingdom (UK) by the Office of National Statistics in 2004 (N = 7,977) was used to examine levels of service access. Children with ID and mental health problems were compared to children with mental health problems (without ID), children with ID and no mental health problems, and typically developing children. Results: Children with ID and mental health problems accessed specialist mental health services at higher rates than children with only ID, but at similar rates to children who only have mental health problems. Conclusions: Overall, low levels of service access highlight likely high unmet need among children with ID, mental health problems, and both conditions. (Publisher abstract)

Older American Indians experience high rates of depression and other psychological disorders, yet little research exist on the depression literacy of this group. Depression literacy is fundamental for individuals seeking help for depression in a timely and appropriate manner. In the present study the authors examine levels and predictors of knowledge of depression symptoms in a sample of rural older American Indians (N = 227) living in the Midwestern United States. Data from self-administered questionnaires indicate limited knowledge of depression and negative attitudes toward seeking help for mental health problems. Additional findings and implications for social work practice and policy are discussed. (Publisher abstract)
Exploration of the dynamic barriers to adults with intellectual disabilities accessing mainstream mental health services

Purpose: The purpose of this paper is to investigate the experiences of psychologists in considering the barriers to people with intellectual disabilities being able to access mainstream mental health services. Design/methodology/approach: The information relating to participant experiences was gathered by conducting two focus groups: one with psychologists working in the adult mental health service and the second with psychologists working in the learning disability service. A thematic analysis was used to determine the main themes from each of the focus groups. Findings: The overarching themes for the psychologists in the adult mental health service related to: service restrictions; confidence in their ability/skills; and resistance to change. The overarching themes for the psychologists in the learning disability service reflected: future thinking; protection; pessimism; and clarification around mental health and learning disability. Originality/value: The discussion includes consideration of the dynamic positions of the two services and recommendations for an integrated system of working.
