Elderly patients and the Mental Health Act 1983

Reports a retrospective casenote review which was carried out in Southport on all patients over 65 years of age who were detained under the Mental Health Act 1983 during the first six years of its operation. There were 132 episodes of detention affecting 120 patients. 53% were for organic illness and 47% for functional illness. the organically ill were detained most commonly because the carer was no longer able to cope or, for those living alone, because of self-neglect. The functionally ill were most commonly detained in the interests of their health alone. The majority of patients with dementia were not discharged to their previous address after being detained but to residential care. Twenty-seven per cent of the whole sample died within a year of being detained, a rate higher than in the general population.

Reports an exploratory study which used tape recorded semi-structured interviews with 31 users attending one psychiatric day hospital. The service users indicated that they saw the day hospital as providing social contact with others, and that it taught them strategies of how to cope with their disorder. Many of them indicated that social isolation from the 'outside world' was their predominate problem. Some appeared to have become reliant on the day hospital for the social contacts it provided.

A study of two strategies of case management Community Treatment Teams (CTTs) and Intensive Case Management Teams (ICMTs). Analysis revealed that although the CTTs engaged in more case management activities, there were few significant differences in the outcomes of the two approaches. The results are discussed in relation to a possible "ceiling effect" of case management, cost effectiveness, and limitations in the sample.

The reform of community and long-term care has been an important theme of policy development around the world during the last decade. British community care is a particularly interesting and impressive example of changes which have parallels in other countries. They include: achieving greater flexibility in the response to the needs and wishes of users and carers; containing costs increases and achieving greater cost-effectiveness; consolidation of structures; competition, variety and choice in a mixed economy; care management and assessment; clarity in targeting; improvement of mechanisms for orchestrating the system.
