Quality of practice in supported accommodation services for people with intellectual disabilities: what matters at the organisational level

Background: Organisational and service level factors are identified as influencing the implementation of Active Support. The aim was to explore differences in organisational leadership and structures to identify potential relationships between these factors and the quality of Active Support in supported accommodation services. Method: Fourteen organisations participated in this mixed methods study, which generated data from interviews with senior leaders, document reviews and observations of the quality of Active Support. Results: Qualitative analyses revealed three conceptual categories: senior leaders in organisations where at least 71% of services delivered good Active Support prioritised practice; understood Active Support; and strongly supported practice leadership. In these organisations practice leadership was structured close to everyday service delivery, and as part of frontline management. Conclusions: Patterns of coherent values, priorities and actions about practice demonstrated by senior leaders were associated with successful implementation of Active Support, rather than documented values in organisational policy or procedures. Predicting good Active Support for people with intellectual disabilities in supported accommodation services: key messages for providers, consumers and regulators

Background: There is strong evidence about the effectiveness of Active Support. Recent research has established predictors of good Active Support as staff training, practice leadership, and service setting size. This paper explores features of organisational leadership and structures predictive of Active Support. Methods: Multilevel modelling with data from surveys, observations and interviews was used to identify predictors of Active Support at the levels of service users (n = 253), services (n = 71) and organisations (n = 14). Results: Good Active Support was predicted by: (1) positive staff perceptions of management, (2) prioritisation of practice and Active Support by senior managers, (3) strong management support for practice leadership, (4) organisation of practice leadership close to everyday service delivery, and (5) concentration of practice leadership with frontline management. Conclusion: These findings extend understanding of predictors of Active Support and provide indicators of service quality, with important implications for service providers, service users and those monitoring the quality of services. National inspection of care and support for people with learning disabilities: overview

Sets out the findings of the national inspection of quality and safety of care and support for adults with learning disabilities. The inspection includes fieldwork in six local authorities and corresponding health boards; the results of a thematic enquiry into services for people with learning disabilities regulated by CSSIW; and the results of a national data and self-assessment survey undertaken in all 22 local authorities in Wales. Inspectors focused on a number areas including: support for carers; whether local authorities provide information, advice and assistance; and whether or not local authorities have good leadership and governance arrangements. Inspectors from Healthcare Inspectorate Wales (HIW) also looked at the efficacy of the partnership between social services and health. Findings of the report looks at what is working well and areas for improvement under the following themes: understanding need, providing effective care and support, and leading in partnership. The report found that the quality of care and support for many people with learning disabilities depended on the effectiveness of the front line social services and health staff and the assertiveness of relatives. The report makes a number of recommendations, including: that local authorities review their quality assurance arrangements for care and support planning with individuals; for local authorities and health boards to share best practice; and for local authorities and health boards to ensure that the lines of accountability and responsibility in relation to adult safeguarding are clear and understood by staff. Competitive tendering of services to non-profit organisations and profit making companies can be very disruptive for staff and service users, especially when involving supported living services for people with learning disabilities. The difficulties people with learning disabilities can face when changing service providers and the concerns that local authorities should address are discussed.

This policy statement provides guidance for registration managers and inspectors assessing providers of services for people with learning disability and/or autism. It aims to ensure the Care Quality Commission (CQC) have a consistent approach to registration of services for people with a learning disability and/or autism and also informs providers, people who use services and their families and carers of the Care Quality Commission (CQC) approach to registration. It applies to three key areas of registration: specialist hospital provision, such as an assessment and treatment unit for people with a learning disability and/or autism; opening a new care home or location; and new applications for registration and applications to change a location’s service type, for example from hospital services to care home or supported living services. Case studies are included to provide examples of applications that are likely to be approved, and application that are unlikely to be granted. Providers of services are more likely to have their application for registration granted if they can demonstrate how their model of support is: is in line with Building the Right Support and the accompanying service model, built on evidence-based care; and is in line with national policy. Consultation document providing details of how the Care Quality Commission proposes to update their approach to regulation and their assessment framework to reflect the changing provider landscape and new models of care. Specifically the consultation seeks views how the CQC will regulate new models of care and complex providers; changes to CQC assessment frameworks across the health and social care services they regulate; how the CQC will register services for people with learning disabilities, including for registered services to comply with the national plan 'Building the right support'; and how CQC will regulate NHS trusts and foundation trusts from April 2017, including changing the approach to rating. Separate annexes provide details of CQC’s assessment frameworks for healthcare services and adult social care – key lines of enquiry and characteristics of ratings and updated guidance on registering learning disability services. One of the 29 recommendations of the 'The same as you?' review was that local authorities and health boards should draw up Partnership in Practice agreements (PiPs). The aim of this national overview report is to provide feedback on the key messages that emerge from a review of all of the 2004-2007 PiPs, taking account of additional evidence from other sources. Key themes identified are: health promotion and improving access to health, Local Area Coordination, carers, short breaks or respite, autism spectrum disorders, Direct Payments, day services, further education and employment, supported living and vulnerable adults. This report aims to highlight positive developments that can be shared to support improvement across Scotland, and to inform the next round of plans for 2007-2010.
