Preventing abuse in accommodation services: from procedural response to protective cultures

This paper considers the dominant policy and practice approaches of Australian disability accommodation services with respect to the abuse and neglect of people with learning disabilities, and questions the effectiveness of these approaches. The paper reviews international literature and provides practice examples to help develop a framework of current research, policy and practice in this area. Findings show that dominant policy and practice approaches do not give adequate consideration to the prevention and protection of people from harm, focusing primarily on responding to individual instances of maltreatment. Managerial, compliance-based systems may be deflecting attention from recognizing and responding more effectively to abuse and neglect at individual, systemic and structural levels. The current dominant approach fails to develop a culture of prevention and protection for people with intellectual disability. The authors concluded that some systemic and structural preconditions are set which make abuse and neglect less preventable.

Purpose: This paper reflects on the response to the scandal of abuse in services for people with intellectual disabilities in the light of research evidence and analysis. Design/methodology/approach: Critical reflection and review of literature. In particular, recent research into possible indicators that a service is at risk of becoming abusive is used to test the hypotheses and implied solutions that are currently being adopted. Findings: The review finds that some of the responses to recent scandals are necessary but not sufficient to prevent future harm. Furthermore, some of the proposed solutions may actually increase the likelihood of further abuse. The paper calls for a broader and more evidence-based response to prevent abuse. The needs of people with learning disabilities who develop dementia: a literature review

The longevity of people with learning disabilities is rising, and they are increasingly developing age related conditions including dementia. When this occurs, their medical and social needs pose many challenges for services. A literature review was undertaken of articles published between 1996 and 2006. Data were collected relating to the needs of people with learning disabilities and dementia, their carers and their peers. Findings revealed that the primary medical need was for timely and accurate diagnosis. There were a multitude of diagnostic tools, and advice was available as to which are most suitable for different client groups. The needs of carers are intertwined with those of people with learning disabilities and dementia and meeting their needs for education, training and increased staff numbers, has proved beneficial. Although multiple services were responsible for the needs of this client group, there was an agreement that learning disability services should be central to social care service provision.
The interface between learning disability and old age psychiatry: two specialties travelling alone or travelling together?

Despite existing policy, there is little guidance on what quality of care and equity of service provision for people with learning difficulties and mental health problems might look like. To address this gap, the Royal College of Psychiatrists set up a group to look at the interface between old age psychiatry and learning disability. It also commissioned a survey of learning disability and old age psychiatrists to investigate their experience of working across the interface between the two specialities - this paper is based on the final report. It presents findings from the survey and sets out eight recommendations to further the work: guidelines on interface working; joint training opportunities; working with social/health care commissioners; development of interface protocols; awareness of other care professionals; development of service models; sharing of expertise; and inclusion of users and carers from the interface area. The authors concluded that, while there were no clear ideas on how services should be provided to older people with a learning disability who also develop a mental health problem, the need for collaboration between the two fields was convincing.
Working on person-centred planning: from amber to green light?

This article reviews the practice and policy based literature on person-centred planning in learning disability services in England. Its aim is to identify the implications for the workforce in practice. The analysis found that implementation is often described as partial or slow and characterizes reasons for this at a number of levels, including the slow pace of change in service culture and power relations, immutable funding structures, services’ inflexible infrastructures, high levels of staff turnover and lack of training, inexperience among service management, inadequate staff supervision, and ambiguity among some stakeholders. Little substantial critique exists of the model itself. Analysis of the literature further reveals that the implementation of person-centred planning in practice is assisted by policy encouragement, service development and investment, favourable case reports and personal accounts, practitioner enthusiasm and positive evaluations. This article explores these to consider what facilitates the adoption of new elements of practice.
Better health and care for all: health and care services for people with learning disabilities

This thematic research review brings together NIHR-funded research on health and social care services for people with learning disabilities and their families. It features 23 recent studies and includes summaries of findings. The review covers the themes: understanding learning disability and services; identifying health risks; keeping well in the community; staying well and safe in hospital and preventing avoidable deaths; services supporting positive behaviour; and conducting research together. It also highlights studies that show examples of good practice in involving people with lived experience of learning disabilities in doing research. It also includes the views of people with lived experience around the issues covered by the research. The research finds that more work is needed to improve care for people with learning disabilities, who experience poorer health and die earlier than the general population. Many of these deaths could be prevented by public health interventions or better access to high quality care. It includes important findings for those who commission, deliver, work in and use these services. (Edited publisher abstract)
Building bridges to a good life: a review of asset based, person centred approaches and people with learning disabilities in Scotland

This review explores the potential to join up thinking on increased choice and control for people with learning disabilities and the principles of asset based working. Commissioned by the Scottish Commission for Learning Disability, it considers the efficacy of asset based approaches for people with learning disabilities, looks at evidence of the impact these approaches can have on people’s lives and also identifies examples of good practice in Scotland. The review draws on the results of a literature review; interviews with key informants involved in asset based working and learning disability services; and a mapping of projects using asset based principles with people with learning disabilities across Scotland. The results suggest that there are is reason why the focus of assets work cannot be broadened to include opportunities for people with learning disabilities. However it suggests that asset based approaches should be seen in the context of efforts to advance the personalisation and social integration agendas, and that if that they need to fit alongside services, support systems and initiatives. Examples included in the review illustrate how services can add to the assets of individuals and communities, provided they are willing and committed to relating to people and doing things differently. Factors identified that facilitate asset based approaches with people with learning disabilities, include: addressing wider inequalities and stigma; ensuring people with learning disabilities are active participants in place based community development; and tackling attitudinal barriers and established ways of doing things.
