Bridging the gap in care provision

There are an estimated 25,000 older people with a learning disability living in England, and this number is rising due to improved care and medical advancements. This article notes that the particular needs of older people with a learning disability are not usually met by standard homes for older people. It describes the work of the Fynvola Foundation and its Lady Dane Farmhouse, a residential home in Faversham dedicated to caring for older people with a learning disability.
Service use and cost of mental disorder in older adults with intellectual disability

The cost of caring for people with intellectual disability, which is already high, may rise as more people with intellectual disability live to old age. In addition the rates of mental illness and dementia are thought to be higher in this group than in the general population. This study reports service use patterns and costs for people (n=212) with intellectual disability (excluding Down syndrome) over 60 years of age living in London. It covered costs of accommodation, health and personal care, physical as well as mental illness, dementia, sensory impairment and disability and aimed to explore the influence of sociodemographic and illness-related determinants. The average weekly cost per older person was £790 (£41080 per year); accommodation accounted for 74%. Overall costs were highest for those living in congregate settings, such as nursing or residential homes. Gender, intellectual disability severity, hearing impairment, physical disorder and mental illness had significant independent relationships with costs. Mental health problems were associated with an additional weekly cost of £202. The authors note that older adults with intellectual disability comprise 0.15–0.25% of the population of England but consume up to 5% of the total personal care budget. They suggest interventions that meet needs and might prove to be cost-effective should be sought.
It’s about time: An overview of short break (respite care) planning and provision in Scotland

This research was carried out for Shared Care Scotland by Reid Howie Associates to look at the planning and provision of short break care in adult and older people’s services. The report presents the findings of research carried out between April and August 2010 to: improve knowledge and understanding of the landscape of respite care and short break provision for adults in Scotland; identify good practice in the planning, commissioning and design of respite care and short break services; and better understand the challenges and identify solutions to moving towards more flexible, personalised respite care and short break services for adults. The research focused on the views of stakeholders in the public sector, although a number of carers’ and other organisations also contributed views.

This paper discusses the paper by Bigby and Frawley in this journal issue on the involvement of a person with intellectual disability as a co-researcher in a large multimethod study. This commentary applauds the project for taking a leap into unknown territory, and then to be so honest in recognising that the intended aims were not met. It points out that the list of requirements normally needed for researchers may not be appropriate for someone with learning difficulties. An approach that might work in involving someone with learning difficulties as a research associate is to design a niche in the project which suits their specific capabilities. This paper goes on to describe some of their own research group’s attempts to involve people with a disability in research into developing a range of assistive technology systems intended to benefit them. In particular it describes the involvement of older users and older users with dementia in iterative design methods to continually comment on and test a series of hopefully improving prototypes.

Carers with learning disabilities are a hidden group within the population, and remain a largely neglected group. The article describes a campaign called ‘Who Cares for Us?' which is working with government and social services to make sure carers with learning disabilities are included in the new carers strategies. The Valuing People Now formed a National Network for Carers with Learning Disabilities, building on the work of 'Who Cares for Us'. The article acknowledges that more needs to be done, but ultimately supporting carers is an invaluable process, both for the community and the economy.

An increasing number of people with learning disabilities are providing regular and substantial care for their ageing relatives. This care ranges from help with personal care, medication, cooking and cleaning, to help with shopping and keeping them company as they go out and about less. In many cases, neither person would be able to remain living independently without this support. This is known as mutual caring. However, mutual caring amongst older families often remains hidden. This booklet highlights some of the main issues facing mutually caring older families and some of the ways that people in different services and roles can make a positive difference. This booklet should be read by anyone who is working in a service or role where they may come into contact with an older family in which a person with learning disabilities is taking on a caring role for their older relative. It focuses on 3 main topics: understanding mutual caring; responding to the challenge of mutual caring; and getting support right for families who are caring for each other.

A number of people with learning disabilities are providing regular and substantial care for their ageing relatives. This briefing note sets out the main findings of a project which conducted in-depth studies with older families that include a person with learning disabilities. The aims of the project included directly supporting the development and promotion of good practice in supporting older families to plan for the future where a person with learning disabilities is providing regular and substantial care to an elderly relative. This briefing note sets out some of the main finding of the project. In particular, it lists guidance and strategies to commissioners and care managers on how to develop an effective service response to these families. A conclusion of the project is that the incidence of mutual caring is growing, but its true extent is not known. Mutual care is often not recognised, and responsibility for these families can often fall between older people’s services, learning disability services and carers’ services.
