Restraint of adults with intellectual disabilities: a critical review of the prevalence and characteristics associated with its use

Background: As part of their care, adults with intellectual disabilities are often subject to restrictive interventions including restraint. Method: A review examining the prevalence of restraint use with people with intellectual disabilities and the characteristics associated with its use. Results: The seven papers identified used quantitative methodologies and had cross-sectional designs. Prevalence rates of restraint ranged from 11% to 78%, multiple forms of restraint were common. Most studies focused on characteristics within the person with an intellectual disability, three considered external factors. Challenging behaviour was the most consistent characteristic associated with the use of restraint, but how this was defined varied. Conclusions: The review highlights a need for more consistent means of defining and measuring restraint and its associated characteristics. Future research into this area may also want to focus on the context of restraint such as whether it is the least restrictive option used. (Publisher abstract)
Symptoms of attention deficit hyperactivity disorder in children and adults with intellectual disability: a review

This article reviews attention deficit hyperactivity disorder (ADHD) as it has been studied in individuals with intellectual disability. Despite a reported excess of ADHD symptoms in individuals with intellectual disability, it has been argued that ADHD symptoms have been under diagnosed and inadequately treated in individuals with intellectual disability. The use of diagnostic criteria for ADHD from international classification systems is discussed with reference to the applicability of these systems to individuals with intellectual disability. The prevalence of ADHD symptoms and correlates of ADHD in individuals with intellectual disability are examined with reference to published studies. The findings show that the reported prevalence rates of ADHD symptoms in individuals with intellectual disability vary significantly depending on instruments and diagnostic practices employed. Published studies evaluating interventions for individuals with ADHD and intellectual disability are also reviewed. These are found to primarily focus on pharmacological interventions. The article concludes that much less is known about ADHD in individuals with intellectual disability than about ADHD in individuals without intellectual disability. There is therefore a need to clarify baseline rates of ADHD in individuals with intellectual disability and to develop interventions to support affected individuals and their families.
Delirium and learning disability: case series and literature review

Although predisposing and facilitating factors for delirium are common in learning disabled patients, little is known about its occurrence. Presents 3 case reports and results of a manual and computer (Medline, Embase Psychiatry) search. Only 2 case reports were found, and added to the 3. Ages were 16, 17, 51, 55 and 78. Delirium was associated with eye surgery, anticholinergic overdose, urinary infection, Alzheimer's disease and pulmonary infection respectively. All had predisposing factors. The literature is scarce, possibly because diagnosis is often missed. Delirium is very distressing and raises morbidity and mortality. However, it is treatable, so more attention should be given to diagnosis and treatment, especially in vulnerable patients. A practice guideline assessment and treatment is given.

Alcohol is the most commonly used psychoactive substance in the general population, and alcohol use disorder is among the most common forms of mental illness. There are a number of additional issues concerning problematic alcohol use that are of particular significance to those with an intellectual disability. This paper presents an outline of the interventions that may be appropriate for an adult with an alcohol use disorder and an intellectual disability. Future research is needed to evaluate the efficacy of interventions with this client group.

A review of the frequency and nature of adaptations to cognitive behavioural therapy for adults with intellectual disabilities

Background: There is increasing evidence that cognitive behavioural therapy (CBT) can be effective for people with intellectual disabilities. The aim of this review was to report the nature and frequency of adaptations reported in studies of CBT for people with intellectual disabilities. Method: This review updated and extended a previous review by (Whitehouse et al. 2006, J Appl Res Intellect Disabil, 19, 55), which used a framework of adaptations developed by (Hurley et al., 1998, J Dev Phys Disabil, 10, 365). Adaptations included simplification, language, activities, developmental level, use of directive style, flexible methods, involve caregivers, transference/countertransference and disability/rehabilitation approaches. A search identified peer‐reviewed papers that reported individual CBT informed psychological interventions for people with an Intellectual Disability. Results: The search identified 23 studies which met the inclusion criteria for review. Studies reported the majority of the categories of adaptation described by (Hurley et al., 1998, J Dev Phys Disabil, 10, 365). Conclusions: The results suggest that the framework of adaptations needs further development to increase sensitivity in identifying and categorizing adaptations of CBT. Increased, systematic reporting of adaptations to studies of CBT with people with intellectual disabilities is recommended. (Edited publisher abstract)
Understanding and treating offenders with learning disabilities: a review of recent developments

This narrative review first presents and discusses recent developments relating to offenders with learning disabilities, looks at the historical association between crime and low intelligence, and then investigates the evidence concerning the prevalence of offending by people with learning disabilities, and their recidivism rates. The authors summarise research concerning service pathways for this population, and outline progress in the development of actuarial, dynamic and clinical assessments of the future risk of offending. Secondly, the paper focuses on a review of the evidence for, and recent developments in, the treatment of offending behaviour (anger and aggression, sexual offending and fire-setting), utilising broadly cognitive behaviourally-based approaches. Finally, the authors propose future directions for research and practice innovation.
Alcohol and illicit drug misuse in people with learning disabilities: implications for research and service development

This paper offers readers a review of the literature on alcohol and illicit drug misuse in people with learning disabilities, focusing on six key areas. First, clarity is provided on the definition of ‘misuse’. Second, prevalence rates are examined along with the methodological difficulties involved in such studies, the authors arguing that prevalence rates are higher than current estimates. Third, the authors explore the relationship between the intra- and inter-personal risk factors. Fourth, the nature of substance misuse is explored, with a focus on offending behaviour. Fifth, a range of treatment modalities are described with a series of recommendations for more robust evidence-based interventions. Last, the authors explore the gaps in policy that lead to a dearth in service provision as well the barriers which people with learning disabilities face on entering treatment services. The paper cites four more innovative projects that address this population’s needs in England, and illustrates how Northern Ireland has positioned the needs of this hidden population within the Department of Health, Social Services and Public Safety (Northern Ireland).
Dual diagnosis among incarcerated populations: exception or rule?

Over 2 million individuals were incarcerated in jails and prisons in the United States in 2004. Multiple studies indicate that the prevalence of mental illness and substance use disorders is substantially higher in correctional environments when compared with rates in the community. The objective of this paper is to provide information on the prevalence of dual diagnosis among those incarcerated and the importance of assessing comorbidity for determining treatment needs of inmates. For the purposes of this article, the definition of dual diagnosis includes mental disorders and coexisting substance use disorders, mental disorders and coexisting developmental disabilities, and developmental disabilities and coexisting substance abuse disorders. An extensive electronic literature search was conducted through PubMed, Medline, Department of Justice, and the National Commission on Correctional Health Care. Studies examining the prevalence of mental illness and substance use in jails and prisons, female inmates, and inmates with developmental disabilities were reviewed. The literature reviewed indicated a high comorbidity of mental illness and substance use disorders in incarcerated individuals. Providers should be aware of issues regarding dual diagnosis in special populations among those incarcerated to include female offenders and offenders with developmental disabilities.
