Supporting adults with learning disabilities to have better lives

This outcomes and improvement framework aims to help directors of adult social services work with their colleagues and partners to identify how they can improve how they support adults with learning disabilities and autism and how they can be assured that the care and support in their area is good value for money. Around 1.04 million people aged 15 to 64 in England have learning disabilities or autism or learning disabilities and autism. This group experience disproportionate levels of inequality and their quality of life outcomes are lower than it is reasonable to expect in the 21st Century. The framework is principally aimed at addressing services for adults with a learning disability and young people in transition from children’s services to adult services. This includes autistic people who also have a learning disability. The framework consists of six domains: provide systems leadership, governance and management arrangements; understand demand and use prevention and early interventions effectively including transitions; enable a care and support system that supports adults with LD&A to have a good quality of life; develop the local workforce to have the capacity and capability needed to support adults with LD&A needs; support adults with LD&A to stay safe, i.e. live the lives they want to; and operate efficient and accessible business systems and processes that ensure value for money. Social services utilization by adults with intellectual disabilities and their families

This study used Andersen's Behavioral Model with the family as a unit of analysis to examine the patterns of awareness and utilization of social services by families with at least one adult with intellectual disabilities. Face-to-face interview questionnaires were conducted with primary family caregivers in Hsinchu City, Taiwan in 2004. Data were collected on adults' and caregivers' predisposing, enabling, and need variables as well as awareness and utilization of formal social services. A total of 792 interviews were completed, making a response rate of 94.2%. Multivariate logistic regression analysis was used to identify potential predictors of social service awareness and utilization. We found awareness and utilization to be mostly associated with predisposing and enabling variables including caregiver's socioeconomic status, family connection with social networks, and family participation in related groups or associations. Younger caregivers were more likely to access formal assistance, while adults whose families were more connected into social networks were more likely to be aware of in-kind and voucher services, e.g., in-home services including respite and home care, and vocational services such as sheltered workshops, and bus voucher services.

Reports on an Anglo-French partnership, between Medway Council and the Maison de l'Initiative in the Grande Synthe region, which is pioneering 'cultural mediation' as a way to combat exclusion among ethnic minorities. The project is funded until July 2005 by the European Union's Interreg IIIA programme. In Medway the project is focusing on improving access to social services for people from ethnic minorities with mental health needs, physical disabilities and learning difficulties. In France the mediators are targeting employment issues for ethnic minorities.
There to help 2: ensuring provision of appropriate adults for vulnerable adults detained or interviewed by police

Based on Freedom of Information Act requests to police forces in England and Wales, this report provides an updated on the need for, and access to, appropriate adults (AA) for vulnerable adults in police custody. Vulnerable adults may have a mental illness, learning disability, brain injury or autism. The report provides information on: the identification of need for an AA amongst adult suspects; the application of the AA safeguard (the extent to which vulnerable adult suspects actually receive the support of an AA); and the availability of organised AA provision in England and Wales. Based on police figures, the report finds that in the 12 months ending 31st March 2018 at least 111,445 police detentions and voluntary interviews of vulnerable adult suspects were carried out without the support of an 'appropriate adult'. It also found that where police had no access to an organised AA scheme, they were half as likely to record an adult as needing one. Despite being a key recommendation of There to Help (2015), local authorities are only legally required to run schemes for children, not for vulnerable adults. A video, made with and for women with learning disabilities in the UK, which describes what domestic violence is and tells women how they can get help. It is based on research which interviewed women with mild to moderate learning disabilities who had been in abusive relationships to explore their experiences and the attitudes and practices of the professionals involved. The video includes interviews with women discussing what they understand about domestic violence. A range of professionals also provide information on the services and support available to women with learning disabilities. These include the police; access to legal advice; help available form healthcare and social services, information about Independent Advocacy Services; and information about the women's refuge Beverley Lewis House in London. 'It's the system working for the system': carers' experiences of learning disability services in Ireland

The aim of this article is to examine the experiences of families with young adults with learning disabilities trying to access services. The landscape of disability services for this group is made up of day care, special vocational training and respite places. It aims to identify the extent of an implementation gap between government rhetoric and the degree to which services are characterised as being non-supportive interactions on the ground. Using Ireland as a case study, during a time when the economy is booming and government rhetoric claims unparalleled developments in allocating resources and extra respite 'places', this article identifies the main challenges faced by family carers associated with accessing appropriate services for their disabled adult child, in their attempt to achieve greater independence. This article reports the findings of a qualitative study in which individual semi structured interviews were held with family carers (n = 25) and representatives from national carer organisations (n = 6) in Ireland. These were people caring for an adult (18–30 years) with a learning disability and their experiences were also useful in cross-checking the carer organisation interviews. The findings show that there is limited flexibility, choice and availability in meeting the preferences of the service-users, and throughout the study, services were characterised as being non-supportive interactions. This is not simply symptomatic of a lack of resources. Despite improved funding, supportive attitudes and flexibility are still crucial in meeting user requirements at the level of delivery; thus highlighting that often the system works for the system, not for the user.
Best value reviews of learning disability services for adults: a framework for applying person centred principles

This framework has been produced for people who are responsible for designing and leading the implementation of Best Value reviews within the learning disability service area. The lives of people with learning disabilities cannot be compartmentalised. Where they live, what they do during their days, what relationships they develop and so on are all inter-linked. It is thus not possible to consider the effectiveness of services concerned with one aspect of their lives without considering others. The framework therefore proposes that reviews should be conducted on the basis of a “whole system approach”, adopting an across the board response to identified needs. The essence of the framework is about how to apply a person centred approach to a review of services. This has the potential to result in more creative outcomes than a traditional service-based review - as such approaches often take the performance of existing service structures as their starting point. Above all, Best Value is about ensuring that fundamental questions are asked about the appropriateness of services, how they are delivered and whether they are meeting needs effectively. This framework offers the potential to do this.

This paper sets out good practice in the approach to commissioning services for people with learning disabilities and/or autism. It is aimed at commissioners, working primarily within health and social care settings, who are responsible for the strategic design and development of local services for people with learning disabilities and/or autism and their families and whose role involves driving innovation and practice change. The paper adapts the IPC’s six steps approach – a model for managing demand in older people’s services, linked to suggested performance indicators and targets – specifically for learning disability services. It includes a set of operational principles, examples of good practice and suggested performance indicators that can be used to form the basis of an effective local commissioning plan that can offer better overall value for the public purse. The six steps approach comprises: information, advice, assistance and advocacy; universal services; universal plus; early intervention; short-term intensive support; long-term specialist support; understanding population and market shaping. Transition: the experiences of young people with learning disabilities and their families in England

Reports on a study of 283 families with youngsters with learning disabilities to examine the experiences of young people and their families in the transition from children's to adult services. The research was carried out by the Home Farm Trust and the Norah Fry Research Centre, University of Bristol, The research consisted of a postal questionnaire which was distributed to 370 families; 27 in-depth interviews which were conducted with parents in the family home; and 27 face-to-face interviews with the young people with learning difficulties from these families. The study found that existing legislation and guidance were largely failing youngsters with learning disabilities and their families at transition. There were substantial discrepancies between what ought to have been provided and what young people and their families experienced in practice, and significant difficulties as youngsters moved between children's and adult health and social services. Other difficulties experienced at transition are also reviewed, along with possibilities for improvements in practice. The study also looked at some innovative schemes and services in this area.
