Quality Action Groups have been set up in different parts of the country as a structured means of bringing 'stakeholders' together to improve services. The author, Quality Assurance Manager at Wiltshire Social Services Department, describes one project and how it achieved greater privacy for a group of people with learning difficulties.

This quality standard covers the care of children, young people and adults with a learning disability and behaviour that challenges. It is focused on ensuring that assessment leads to personalised care planning and access to meaningful activities. It consists of eight quality statements, aiming to ensure that the approaches used by staff to support people with a learning disability follow the least restrictive practice and promote privacy and dignity. The statements cover: comprehensive health assessment; initial assessment of behaviour that challenges; designated coordinator; personalised daily activities; review of restrictive interventions; use of medication; review of medication; and family and carer support. An earlier report by NDTi, ‘Reasonably adjusted’ (2012) described the reasonable adjustments mental health services were already putting in place for people with learning disabilities and people with autism. The NHS Confederation, supported by the Department of Health, commissioned the NDTi to develop and produce materials to help services review their own quality and share and replicate good practice. These are published as the Green Light Toolkit 2013, which comprise practical new materials designed to help improve the quality of mental health services for adults with learning disabilities and/or autism. The toolkit includes an audit framework to support reviews; an easy-read version of the audit framework and toolkit; and examples. (Edited publisher abstract)

There are approximately 1.5 million people in Britain living with learning disabilities, and the number is likely to grow by 14% per cent between 2001 and 2021. The UK Modernising Learning Disabilities Nursing Review wants to ensure that people with learning disabilities of all ages have access to the expert learning disabilities nursing they need. This report seeks to ensure that people with learning disabilities of all ages have access to expert Learning Disabilities Nurses and that their families and carers get the best support and care. It also seeks to make best use of Learning Disabilities Nurses throughout the entire health and social care system and improve the career image of learning disabilities nursing as a whole. It sets out a number of detailed recommendations for the 4 UK Governments aimed at strengthening the commitment to learning disabilities nursing across the public sector. Underlying the recommendations are 4 organising principles for supporting reform: strengthening capacity through developing the learning disabilities nursing workforce; strengthening capability to ensure a competent and flexible learning disabilities nursing workforce; strengthening quality and demonstrating quality outcomes; and strengthening and modernising the profession.

Service users get together with a social care academic and a leader in the home care sector to discuss how excellence can be achieved in domiciliary care. We see examples of excellence in support offered to older people, people with learning disabilities and people with physical disabilities in their own homes. Much of the focus is on the relationship between staff members and the people they support because getting that right is fundamental to excellent care. This film has now been revised. Initial findings from a provider collaboration review exploring the care for people with a learning disability who live in the community, and what impact the COVID-19 pandemic has had on them and the services they receive. Many of the issues emerging from review are not new. In a lot of cases, the pandemic has simply served to shine a light on pre-existing challenges, gaps and poor-quality care. There are some approaches to care delivery that can provide people with the care that they need, in a way that enables them to lead their best lives. For example: giving people choice, control and independence; access to the right care and support at the right time; and collaboration between services, and with the person and their families. The findings of this provider collaboration review support our wider ambitions to improve our regulation of services for people with a learning disability and autistic people. Supporting adults with learning disabilities to have better lives

This outcomes and improvement framework aims to help directors of adult social services work with their colleagues and partners to identify how they can improve how they support adults with learning disabilities and autism and how they can be assured that the care and support in their area is good value for money. Around 1.04 million people aged 15 to 64 in England have learning disabilities or autism or learning disabilities and autism. This group experience disproportionate levels of inequality and their quality of life outcomes are lower than it is reasonable to expect in the 21st Century. The framework is principally aimed at addressing services for adults with a learning disability and young people in transition from children’s services to adult services. This includes autistic people who also have a learning disability. The framework consists of six domains: provide systems leadership, governance and management arrangements; understand demand and use prevention and early interventions effectively including transitions; enable a care and support system that supports adults with LD&A to have a good quality of life; develop the local workforce to have the capacity and capability needed to support adults with LD&A needs; support adults with LD&A to stay safe, i.e. live the lives they want to; and operate efficient and accessible business systems and processes that ensure value for money. This statutory guidance applies to any service that currently, or intends to, provide regulated care to autistic people and people with a learning disability, including children and young adults, working age adults and older people. It describes the CQC regulatory approach for these services, covering registration, inspection, monitor and enforcement functions. Key requirements providers are expected to demonstrate include: there is a clear need for the service and it has been agreed by commissioners; the size, setting and design of the service meet people’s expectations and align with current best practice; people have access to the community; the model of care, policies and procedures are in line with current best practice.
