Comments on an article in this issue of Tizard Learning Disability Review. The authors draw on their own research mapping the progress of hospital closures in Scotland and argue that progress remains slow. They find there is a long way to go before recommendations of the Scottish Executive publication 'The same as you?' will be met.

This report investigates access to independent advocacy services for people with mental health problems, learning disability, dementia and related conditions in Scotland. It is based on information received from health boards, local authorities, and from the new health and social care partnerships (HSCPs). The report also investigates whether the integrated children’s services plans of local authorities cover the provision of independent advocacy services for children and young people with mental illness, learning disability or related conditions. The results show that the planning and provision of advocacy services across Scotland is variable and lacks clarity. It found services for adults were often prioritised for those who were receiving compulsory care or treatment .There is a lack of clarity on which organisation is responsible for co-ordinating the preparation of strategic advocacy plans, and on how advocacy providers and people using advocacy services will be involved in planning. The report identifies significant gaps in service provision for children and young people, with some of the services not providing independent advocacy and very restricted eligibility criteria. This review seeks to address the health needs of all members of the population and reduce inequalities. People with learning difficulties frequently have complex heath, social and educational needs that demand high levels of support. Failure to recognise an meet their needs often leads to their social exclusion. It is therefore necessary that actions required to improve health and reduce inequalities are supported by strategic planning resourcing, knowledge and expertise.

This article deals with the resettlement of people with learning difficulties from Scottish hospitals. First it outlines the policy background, tracing the rise and demise of the long-stay institution. It then reports on a study examining current resettlement policies in Scotland, where there is no national closure programme. The research found that resettlement is largely in disarray, with particular difficulties in planning, financial arrangements and inter-agency collaboration. Argues that there is a great risk of hospitals being perpetuated or reconstructed, particularly through the activities of NHS Trusts, and of people with dual labels remaining resident in them for years to come.
