Program development of reminiscence group work for ageing people with intellectual disabilities

The goal of this study was to adapt a narrative reminiscence program for the special needs of ageing people with mild/moderate intellectual disabilities. Research has shown that stimulating reminiscence in the elderly can be a meaningful activity, and holds promise for positive effects on well-being. In the first stage (10 weeks), the program was developed with 1 group. Evaluation and adjustments to the program were made based on video recordings, the researcher's log, and feedback from participants and support workers. Formative evaluation was performed by means of a within group analysis. In the second stage (3 weeks), the program was introduced to 6 other groups. Interviews with professional support workers were subjected to a cross-case analysis. The final program consists of 13 sessions covering different reminiscence themes. The success of reminiscence group work relies on (a) well-prepared and structured sessions, (b) adequate use of visual triggers, and (c) facilitating, coaching and moderating. The program was perceived as a valuable and meaningful activity by all participants. Although reminiscence group work is not therapeutic in nature, it may have therapeutic use for ageing people with intellectual disabilities, and in this sense is worth evaluating.

Dementia is a particular concern for service providers supporting older people with learning disabilities. Until recently little forward planning had been built into services to prepare for residents to grow older, and cope with medical conditions of older age such as arthritis, osteoporosis or dementia. For people with Down’s syndrome, there is a high prevalence rate of dementia as medical advances mean they, as with the rest of the population, are living longer lives. Little is known about the views and experiences of people with a learning disability who have dementia or are living with someone with dementia in a residential setting (Wilkinson et al 2003, 2004; Lyngaard 2004). A three year research project funded by the Community Fund in collaboration with CARE1 has explored best practice for people with a learning disability who develop dementia - looking at what enables people to remain in their own homes and communities. A key element of this wider project was a service user advisory forum. One of the things this research wanted this group to tell us was what people with learning disabilities know about dementia and what it was like to live with someone who has dementia.
Life story work in health and social care: systematic literature review

Life story work (LSW) has been used with a range of client groups, and is viewed as a way of challenging stereotypical views, helping to maintain personal identities and improving the quality of care by giving staff a better insight into individuals. This review of 14 research and evaluation studies (dealing solely with older people and employing a range of methodological approaches) finds that LSW can achieve these objectives, but can also have limitations as well as resource implications. The existing evidence base is patchy, of variable methodological quality and difficult to assess effectively with currently available critical appraisal tools. Research gaps are identified. The review concludes that LSW has the potential to be a valuable intervention but is a complex activity that should not be implemented on a large scale in the absence of a well thought out strategy that includes staff support and training, and the development of protocols for obtaining consent and agreeing ownership and access to the ‘product’.
Mild cognitive impairment: a ‘hardening of the categories’?

Mild cognitive impairment (MCI) is a controversial label for forms of age-related, intellectual difficulties unassociated with impairments in activities of daily living. The principal issue is whether this arbitrary, heterogeneous and unreliable term is appropriate to use clinically. Persons with the label MCI may progress to different types of dementia, stay stable, or even improve. In this article we offer a philosophical analysis of MCI from epistemological, ethical and semantic perspectives. The authors believe that the term should currently not be used clinically.

Produced by NICE and SCIE, this guideline makes recommendations for the identification, treatment and care of people with dementia and the support of carers within primary and secondary healthcare, and social care. The guide is for health and social care staff who work with people with dementia and their carers, and those who work with older people and people with learning disabilities. This includes GPs, nurses, geriatricians, psychiatrists, social workers, care home managers and care staff. It also includes recommendations relevant to commissioners, managers and coordinators of health and social care.

Holiday breaks for adults with intellectual disabilities living with older carers

The evaluation of a project instigated by a voluntary group provided an opportunity to investigate the benefits of holiday breaks to both carers and participants. Feedback was obtained from over 100 people with intellectual disabilities through individual and group interviews, as well as from their carers using self-completed questionnaires. Overall, the breaks were thoroughly enjoyed by the project members, providing them with a range of activities and leisure pursuits in the company of their friends and, on certain breaks, with non-disabled people as well. Carers reported more benefits to their relative in the later years of the project and became more willing for their relative to attend. Three main issues are discussed: the use of mainstream settings and services, rather than specialist facilities, to provide short breaks; how best to encourage other carers of people with more significant needs to avail themselves of these opportunities; and ways of improving the social and leisure networks of people living with families so that the benefits of the ‘holiday’ experience could be recreated more locally and more frequently.

The Government has introduced benefit reforms that permit the disabled, single mothers and older people claimants to try out work, and to return to protected levels of benefit more easily if a job doesn’t work out. For those who are considered more work ready such changes make a real difference. This easy read book sets out their rights and obligations. However, fears persist among claimants that they may fall outside of the protection afforded by such rules or that their continued incapacity might be drawn into question if they try out work.

Netborough: integrated service networks to meet the needs of older people and people with learning disabilities

This paper examines how integrated service networks could be used to provide personalised, outcome-focused services. Using as an example the fictitious ‘Netborough’, the paper shows how local partnerships might set out to meet the challenge of improving services of older people and people with learning disabilities. It describes how networks are organised, the arrangements for care management and support brokerage, and the implications for commissioning, market management and governance. ‘Netborough’ is invented as a way of setting out and drawing together the strands of thinking and requirements in the green paper ‘Independence, Well-being and Choice’, and the white paper ‘Our health, our care, our say’, and is designed to stimulate discussion and debate amongst people working in or concerned with, service provision.
